It is maximized at the cut-off point that optimizes the biomarker's overall classification rate while assigning equal weight to sensitivity and specificity. In this paper, we consider the problem of estimating the Youden index when only group-tested data are available. The unavailability of individual disease statuses poses a challenge, especially when there is differential false positives and negatives in disease screening.
