Endoftalmitis posquirúrgica con meningitis secundaria. A propósito de un caso
A 61-year-old man exhibited hyperacute endophthalmitis due to Proteus mirabilis after a pars plana vitrectomy. During the index examination (24 hours after surgery), he showed visual acuity (VA) of hand motion, biomicroscopy revealed the presence of an edematous cornea, Tyndall++++ and fibrin membrane, with vitritis and impossibility of visualizing retinal details. Treatment with intravitreal injections was administered. Despite therapy, the patient's symptoms worsened, and his general condition started worsening with, fever, and leukocytosis, requiring hospitalization and IV therapy due to secondary meningitis following postoperative endophthalmitis. The patient required enucleation of the affected eye eventually without prosthesis placement.
Even though postoperative bacterial endophthalmitis is usually an infection confined to the eye, this case report demonstrates the infection can spread to the rest of the body, thus becoming life-threatening for the patient.
