This report presents a rare instance of unintentional Datura stramonium poisoning in a 3year-old Ethiopian toddler.</p><p><strong>Case presentation: </strong>He presented with symptoms consistent with anticholinergic toxidrome, including altered mental status, agitation, seizures, dilated pupils, and tachycardia. Laboratory tests, including complete blood count, liver function test, renal function test, electrolyte levels and cerebrospinal fluid analysis were normal. Results of blood toxicology screen were negative.
