Reconstruction of auricular defects after skin cancer excision remains a vexing surgical challenge owing to the complex three-dimensional- anatomy of the external ear, particularly in the conchal bowl. The postauricular island \"revolving door\" flap is a reliable single-stage method for reconstructing anterior auricular and conchal defects. While the revolving door flap is well known in the literature, a clear description of how to use the flap to reconstruct an anterior auricular conchal bowl defect is lacking.
