A possible new, simpler classification was proposed by the lead author and discussed with the co-author. A simplified classification is presented with rationale, mainly based on the simplified treatment strategies used by the authors. We also discuss the possible drawbacks and call for investigations on this topic to make the current treatment strategies less complex.
