Access to current information is imperative for informing clinical strategies and public health initiatives targeting NETs.</p><p><strong>Methods: </strong>A retrospective, population-based study analyzed NET patient data from 1975 to 2020, using the Surveillance, Epidemiology, and End Results (SEER 8, 12, 18) program. We calculated annual age-adjusted incidence, prevalence, and 5-year overall survival (OS) rates. Survival trends from 2000 to 2019 were examined, employing the Fine-Gray model to evaluate cancer-specific mortality.</p><p><strong>Results: </strong>NETs' age-adjusted incidence rate quadrupled from 1.5 per 100,000 in 1975 to 6.0 per 100,000 in 2020. A decline in incidence occurred from 6.8 per 100,000 in 2019 to 6.0 per 100,000 in 2020. All-cause survival multivariable analysis demonstrated high grade (HR: 2.95, 95% CI: 2.63-3.09, P <0.001), single patients (HR: 1.49, 95% CI: 1.45-1.54, P <0.001), and Black patients (HR: 1.17, 95% CI:1.13-1.22, P <0.001) all had worse survival than their controls.</p><p><strong>Conclusions: </strong>Our study shows a steady increase in NETs incidence until 2019, with a decline in 2020. Understanding the reasons behind this trend is vital for improved management and public health planning.
