Legg-Calvé-Perthes Disease: Diagnosis, Decision Making, and Outcome.

Abstract: Legg-Calvé-Perthes disease (LCPD), or idiopathic avascular necrosis of the proximal capital femoral epiphysis in children, has a variable presentation and can result in significant femoral head deformity that can lead to long-term functional deficits. Plain radiographic imaging is crucial in diagnosing LCPD and guiding treatment. Although the etiology of LCPD remains unknown, the evolution of the disease has been well characterized to include the phases of ischemia, revascularization, and reossification. The mechanical weakening during these phases of healing place the femoral head at high risk of deformity. Treatment of LCPD, therefore, focuses on minimizing deformity through operative and nonoperative strategies to reduce the risk of premature osteoarthritis. Advanced imaging using perfusion MRI may refine surgical decision making in the future, and biological treatments to improve femoral head healing are on the horizon.

As an official clinical journal of the American College of Sports Medicine (ACSM), Current Sports Medicine Reports is unique in its focus entirely on the clinical aspects of sports medicine. This peer-reviewed journal harnesses the tremendous scientific and clinical resources of ACSM to develop articles reviewing recent and important advances in the field that have clinical relevance. The journal’s goal is to translate the latest research and advances in the field into information physicians can use in caring for their patients. To accomplish this goal, the journal divides the broad field of sports medicine into 12 sections, each headed by a physician editor with extensive practical experience in that area. The current sections include: Head, Neck, and Spine - General Medical Conditions - Chest and Abdominal Conditions - Environmental Conditions - Sideline and Event Management - Training, Prevention, and Rehabilitation - Exercise is Medicine- Nutrition & Ergogenic Aids - Extremity and Joint Conditions - Sport-specific Illness and Injury - Competitive Sports - Special Populations
