This case report presents a 61-year-old male who reported to the emergency department with sudden abdominal pain and vomiting, resembling a previous episode experienced a month prior. Despite a lack of significant medical or trauma history, the patient's clinical examination revealed signs of internal bleeding. Imaging studies, including ultrasound and contrast-enhanced CT scan, confirmed an enlarged and disrupted spleen. Hemoglobin levels further supported the diagnosis, along with the retrieval of bloody ascitic fluid during paracentesis. The patient remained clinically stable throughout observation and vigilant monitoring.
