It usually occurs in middle-aged women with few or no cardiovascular risk factors. Coronary angiography can make the diagnosis in most cases, and endocoronary imaging is reserved for ambiguous cases. There is currently no randomized controlled trial comparing the different management strategies for spontaneous coronary dissections (conservative approach, coronary revascularization strategies including percutaneous coronary intervention and coronary artery bypass grafting). 摘要

自发性冠状动脉夹层(Spontaneous coronary artery dissection, SCAD)是一种急性冠状动脉事件,与中膜发生血肿有关,导致内膜或内膜-中膜复合物与下层血管分离,从而压迫真管腔,引起缺血和心肌梗死。它通常发生在很少或没有心血管危险因素的中年妇女。冠状动脉造影可对大多数病例作出诊断,而冠状动脉内显像则保留给模棱两可的病例。目前尚无随机对照试验比较自发性冠状动脉夹层的不同治疗策略(保守入路、经皮冠状动脉介入治疗和冠状动脉旁路移植术等冠状动脉血运重建术)。因此,目前的做法是基于病例观察和临床经验登记,以及对急性冠状动脉综合征治疗指南的推断(如果适用)。
