Microscopically, the primary lesions found in the heart were cardiomyocyte hypertrophy and necrosis, myocytolysis, and the presence of myriad basophilic bacteria. Liver fragments and endocardial swabs were sent for bacterial culture, in which pleomorphic Gram-positive rods grew, forming small and hemolytic colonies.
