It presents commonly as an incarcerated inguinal hernia and intraoperative diagnosis is usually the rule. This is a report of a 7 months old male infant presenting with bilateral inguinal hernia, including a complicated left AH, with associated mobile caecum and meatal stenosis. He had bilateral herniotomy, transhernial appendectomy and meatotomy and did well post-operatively.
