Traditional AVR via median sternotomy, poses considerable risks for patients with comorbidities or advanced age.</p><p><strong>Objective: </strong>This study evaluates the efficacy and safety of mini sternotomy aortic valve replacement (MSAVR), a minimally invasive technique designed to reduce surgical trauma, improve postoperative recovery, and lower healthcare costs.</p><p><strong>Methods: </strong>A reversed L-type upper partial sternotomy (RLUPS) approach was performed in five patients focusing on standardized surgical procedures, and rigorous postoperative care. Key findings indicate that MSAVR markedly reduces postoperative complications, ICU stay, and overall hospital stay compared to traditional sternotomy.</p><p><strong>Results: </strong>The median postoperative length of stay was seven days for MSAVR versus 12 days for traditional sternotomy, with ICU stays of 52 hours and 119 hours, respectively. Our study also highlights the cost-effectiveness of MSAVR, with decreased hospital costs per patient due to reduced ICU resource utilization and shorter hospital stays. However, it can also manifest in extranodal sites, including rare instances of orbital involvement.</p><p><strong>Objective: </strong>This report aims to present atypical symptoms associated with HL and its orbital manifestations in a pediatric patient.</p><p><strong>Case report: </strong>To discuss the clinical presentation, diagnostic journey, and management of a 4-year-old boy who exhibited orbital manifestations of HL. Initially, he was misdiagnosed as having an infectious process due to symptoms such as ptosis (drooping eyelids) and eyelid edema (swelling). Consequently, the patient received inappropriate treatments that delayed the correct diagnosis. This case highlights the challenges posed by atypical presentations in pediatric patients and underscores the importance of considering malignancy in the differential diagnosis. A lot of unbalanced translocations have been reported.</p><p><strong>Objective: </strong>Here, we present an individual who has an unbalanced translocation caused by deletion of the terminal region of chromosome 5p encompassing 170 kb coupled with a 11.4 Mb duplication of the terminal region of chromosome 18q..</p><p><strong>Case presentatioin: </strong>We report the first case of unbalanced translocation in a phenotypically normal male after performing clinical phenotyping, cytogenetic analyses and then array-comparative genomic hybridization after detection of unbalanced translocation in his fetus. Conventional G-banded karyotyping showed additional chromatin of unknown origin on the long arm of chromosome 5: 46,XX,add(5)(p15.3). The microarray result confirmed an unbalanced translocation with the loss of ~170kb of chromosome 5p and duplication of 11.4Mb of the long arm of chromosome 18 (arr[GRCh37]5p15.33(22149_192836)x1, 18q22.1q23(66590438_78012829)x3 of a normal adult.</p><p><strong>Conclusion: </strong>This is the first time we found the unbalanced translocation in a totally healthy man to our knowledge. Intracameral and topical antibiotics are currently used to prevent endophthalmitis after cataract surgery. When applying topical antibiotics, there are different recommendations on the frequency and duration of therapy. The development of bacterial resistance to the excessive and long-term use of antibiotics is a growing problem worldwide. The goal is to achieve a good antibiotic effect with the shortest possible use of antibiotics.</p><p><strong>Objective: </strong>The aim of this study was to compare the effectiveness of a new combination therapy of dexamethasone and levofloxacin for seven days after cataract surgery with the previous regimen of dexamethasone, neomycin sulfate, and polymyxin B, which was given for 21 days.</p><p><strong>Methods: </strong>A retrospective analysis of medical records and administered a questionnaire was conducted to assess the effectiveness of postoperative therapy in our cataract surgery patients. The study involved 52 patients who underwent surgery within the last year, performed by a single surgeon at our institution. The findings can help us improve the quality of care we provide and optimize our patients' overall quality of life.</p><p><strong>Results: </strong>We conducted an in-depth study on 52 individuals who underwent cataract surgery at our institution. The prescribed therapeutic regimen for the participants included administering Ducressa solution four times daily for the first seven days and Maxidex solution three times daily for the subsequent 14 days. The study found that none of the participants experienced complications after surgery, and all found it easy to instill the medication. The majority of these adolescents experience unintended pregnancy ending in abortion. Knowledge gaps and misconceptions about reproductive health are the main reasons for unintended pregnancy among adolescents.</p><p><strong>Objective: </strong>This study aims to identify knowledge, attitudes, practices, and related factors of reproductive health among adolescent post-abortion or those seeking abortion at Hanoi Obstetrics and Gynecology Hospital (HOGH), a tertiary hospital in Vietnam.</p><p><strong>Methods: </strong>Ours was a descriptive cross-sectional study of 103 adolescents who sought induced abortions between January 1, 2022 and June 30, 2023. Participants were interviewed directly via questionnaires to collect information.</p><p><strong>Results: </strong>The mean age of participants was 16.3 years. 64.1% of the population did not have general knowledge regarding reproductive health, 42.7% of subjects displayed incorrect attitudes regarding reproductive health. As a result, lack of knowledge and incorrect attitudes led to unsafe sex. The percentage of adolescents practicing unsafe sex is incredibly high (90.3%) thus causing unintended pregnancies. Education levels and family economic status were the main factors linked to knowledge, attitudes, and practices (KAP) regarding reproductive health.</p><p><strong>Conclusion: </strong>Most adolescents seeking abortion had poor KAP regarding reproductive health. Their KAP of reproductive health were linked to levels of education and family economic status. The findings emphasize the need to provide reproductive health care information and services for adolescents, and the need for appropriate attention from both family and society to the target group. {"title":"Prevalence of Pulmonary Embolism in COVID-19 at Quaternary Hospital Running Head: Pulmonary Embolism in COVID-19.","authors":"Jose Maria Pereira de Godoy, Aline Giovana Dizero, Marcia Valeria Caldeira Angelucci Lopes","doi":"10.5455/medarh.2024.78.146-148","DOIUrl":"10.5455/medarh.2024.78.146-148","url":null,"abstract":"<p><strong>Background: </strong>Patients with COVID-19 are at greater risk of pulmonary embolism.</p><p><strong>Objective: </strong>The aim of the present study was to evaluate the monthly prevalence of pulmonary embolism diagnosed by angiotomography and mortality between March 2020 and May 2021 in more than 6000 patients hospitalized with COVID-19 at a single institution.</p><p><strong>Methods: </strong>A clinical trial was conducted with evaluated medical records the patients hospitalized at the institution who developed pulmonary embolism determined by angiotomography. Monthly and overall mortality rates between March 2020 and May 2021 in this population were evaluated.</p><p><strong>Results: </strong>A total of 6040 patients were hospitalized in this period, 203 of whom (3.36%) had an angiotomographic diagnosis of pulmonary embolism and 119 of these patients (58.62%) died. The largest number of patients with pulmonary embolism occurred in the periods from July to September 2020 and March to May 2021. No significant difference was found between mortality and the two peaks of the pandemic (p = 0.9, Fisher's exact test).</p><p><strong>Conclusion: </strong>Pulmonary embolism is associated a higher mortality rate among patients with COVID-19. Carbonic Circumcision is a surgical procedure which has the potential to be performed in a public health setting and offers several advantages particularly in the prevention of disease.</p><p><strong>Objective: </strong>This study aims to evaluate the financial, medical, and sociodemographic impacts of delayed circumcision as compared to early circumcision.</p><p><strong>Methods: </strong>This retrospective study was carried after obtaining IRB approval from our institution (IRB-2024-01-814). Records of circumcision cases from January 2020 to January 2024 were reviewed comparing early circumcisions (within the first 90 days of life) done under local anesthesia with delayed circumcisions (after 180 days of life) done under general anesthesia.</p><p><strong>Results: </strong>A total of 192 patients were excluded due to various reasons: Hypospadias (57 cases), ambiguous genitalia with bilateral undescended testes (26 cases), penile web (22 cases), buried penis (12 cases), penoscrotal transposition (4 cases), and other systemic abnormalities (7 cases). Studies have shown multiple long term economic benefits when circumcision is done early on in life. The cost of circumcision performed later in life is relatively higher than that performed early. In older children, circumcision requires general anesthesia, hospital bed utilization and post operative care which significantly increases the direct cost of the procedure compared to a procedure that is done in an outpatient setting under local anesthesia.</p><p><strong>Conclusion: </strong>Delayed circumcision translates into expedited healthcare costs while circumcising neonates fosters more overall affordable and less risky surgery. When coupled with internal or lumbar injuries, this forms \"Seat Belt Syndrome.</p><p><strong>Objective: </strong>This report details the case of a young male involved in a frontal collision, presenting with severe multiple complicated injuries including abdominal trauma with multiple damage control surgeries.</p><p><strong>Case presentation: </strong>The patient presented to the emergency department after road traffic accident with a seat belt sign, Exploratory laparotomy unveiled extensive injuries, including complete jejunal transaction, avulsion of the terminal ileum, sigmoid mesentery avulsion, and rectosigmoid junction degloving with left anterolateral muscle destruction forming a traumatic hernia. Surgical interventions included jejunal resection and anastomosis, ileocecal limited resection and anastomosis, lastly Hartmann procedure and left high colostomy creation. Subsequent procedures addressed complications, in form of anastomotic leakage managed with resection of the ileocolic anastomosis and ileostomy creation, Vacuum-Assisted Closure dressing, and biological mesh placement to manage abdominal wall infection and dehiscence. After one year the patient underwent surgery for ileostomy and colostomy reversal, along with abdominal wall reconstruction using biosynthetic mesh.</p><p><strong>Conclusion: </strong>Managing injuries involving the jejunum, ileum, and rectosigmoid requires careful surgical intervention, considering individual cases and surgeon judgment. It is a rare soft tissue tumor with poor prognosis.</p><p><strong>Objective: </strong>We report a case of Malignant Triton Tumor (MTT) arising in the right shoulder in a 46 year old male patient presented to our Musculoskeletal Oncology Clinic at Royal Rehabilitation center at King Hussein Medical Center during June 2018.</p><p><strong>Case presentation: </strong>The patient was complaining of an 8 months long progressive right shoulder pain and swelling at the posterior lateral area of the shoulder. It is due to the reactivation of the dormant virus in the sensory ends that were present since the primary infection with the virus.</p><p><strong>Objective: </strong>Our study aims at assessing the knowledge, attitudes practices beliefs and current barriers towards the Shingrix vaccine to fully understand where we stand and come up with proper recommendations to services these at-risk individuals and ultimately have a positive impact on the healthcare system.</p><p><strong>Methods: </strong>Questionnaire based cross sectional study using a translated version of a previously developed and tested questionnaire.</p><p><strong>Results: </strong>more than 8000 participants responded to the survey. The vast majority had chicken pox before and did hear about the vaccine from different sources.
