Infection in the PPS may cause a series of life-threatening conditions such as mediastinal abscesses. Herein, we present the case of a PPS abscess involving multiple maxillofacial spaces, which was successfully incised and drained using a minimally invasive transoral approach under local anaesthesia. This minimally invasive approach requires full anatomic knowledge of the neck spaces. Over the years, selection criteria have moved from profound to severe to moderate hearing loss. Now those with good residual low-frequency hearing, or only a unilateral hearing loss can benefit. Several exciting advances have been made in the last few years. A combination of technological advancements in device hardware, processing capabilities, improved surgical techniques and electrode factors, such as correct scala placement and modiolar proximity were found to correlate positively with patient outcomes. This is an important consideration as the electrode forms a permanent and lifelong interface between the cochlear implant delivering stimulation and the neural structures of the cochlea. {"title":"The effect of mouthwash containing chlorhexidine digluconate 0.2% on halitosis in smokers and non-smokers","authors":"G. Gavazova, P. The aim of this study is to investigate halitosis after administration of mouthwash containing chlorhexidine digluconate 0.2% (Parodontax, GlaxoSmithKline, GB). Material and methods: The study included 60 dental patients, 30 smokers and 30 non-smokers, all clinically healthy, who had not used mouthwash containing chlorhexidine digluconate 0.2% for the past 6 months. The patients were instructed to rinse their oral cavity with the supplied mouthwash twice daily for a 14-day period and to maintain oral hygiene as usual. The rinsing technique involved swishing with 10 mL of chlorohexidine digluconate 0.2% twice, successively, for 1 minute. After rinsing, the mouth was not flushed. Halitosis of the air exhaled from the oral cavity was measured using FitScan Breath Checker HC-212SF (Tanita Corporation, USA) during normal breathing, in the morning, on an empty stomach, without oral cavity toilette - before the start of the study and on day 14 from the start of administration of mouthwash containing chlorhexidine digluconate 0.2%. Results: There was no statistically significant difference between the two sexes in terms of age (p = 0.392). There was no statistically significant difference in halitosis levels between smokers vs. non-smokers (p = 0.07). In terms of sex distribution of smokers, 15 (39.5%) of the female subjects and 15 (68.2%) of the male subjects used tobacco products. A statistically significant difference was found between the two sexes in terms of smoking (p = 0.032). Fourteen-day administration of Parodontax resulted in a statistically significant decrease in halitosis levels in the subjects studied - 2.1 ± 1.15, p < 0.0001. This trend persisted in both smokers (the mean halitosis level after a 14-day period of administration of Parodontax was 2.27 ± 0.21, p = 0.006) and non-smokers (the mean halitosis level after a 14-day period of administration of Parodontax was 1.93±0.21, p < 0.0001). {"title":"Successful management of extramedullary plasmacytoma of the larynx in a patient with advanced radiation therapy techniques","authors":"Mark A. D' Andrea, G. The most affected sites of EMP are primarily the head and neck regions. In this study, we report a case of this rare neoplasm in the larynx and its management using an advanced radiation therapy protocol. Methods: A 42-year-old Hispanic female presented with a history of progressive hoarseness for several weeks. Initially the patient underwent endoscopic evaluation of her throat to find any tumour lesion in the region of her false vocal cord. The patient was diagnosed with the presence of EMP of the larynx using microscopic laryngoscopy followed by computed tomography (CT)/positron emission tomography (PET)/magnetic resonance imaging (MRI) and pathological evaluations. Results and discussion: The findings of the endoscopic evaluation and microscopic laryngoscopy of the patient’s throat indicated a submucosal mass in the region of her false vocal cord that was keeping the true vocal cords from adequately apposing. The CT/PET/MRI and pathology evaluations revealed the presence of a stage I EMP of the larynx. Following the diagnosis of her disease, the patient was administered a course of conformal 3-Dimentional intensity modulated radiation therapy (IMRT)/image-guided radiation therapy (IGRT) to a total dose of 5,040 cGy in 28 fractions over 44 calendar days. A post-operative treatment evaluation after more than 7 years indicated no evidence of a recurrent neoplasm or distant metastasis. {"title":"Functional endoscopic sinus surgery (FESS): Is it always a safe procedure?","authors":"E. We never had major post-FESS complications up to 6 months ago. Therefore, we wanted to compare our results with those of Literature. During the period 2013-2018 we performed 589 FESS interventions. We had a percentage of minor complications similar to reported in the Literature (5-10%), and a lower incidence of major complications (0.34%). We therefore look in detail at our cases of major complications, with the support of preand post-operative imaging. We believe that it is essential to carefully study preoperative imaging. Besides operating in a standardized manner, despite the variation of anatomical features, it is the best way to prevent complications of a FESS. In a department of Otorhinolaryngology without a high-volume series like ours, the complication percentages derived from FESS are low and comparable with those of Literature.
