{"title":"Immunomodulatory Strategies for Spinal Cord Injury.","authors":"Jonghyuck Park","doi":"10.26717/bjstr.2022.45.007202","DOIUrl":"https://doi.org/10.26717/bjstr.2022.45.007202","url":null,"abstract":"<p><p>Neuroinflammation is a key secondary event after spinal cord injury (SCI) and can increase barriers to regeneration, leading to various neurological disorders. Infiltrated hematogenous innate immune cells into the injured site are considered the main effector cells of the inflammatory responses after SCI. Glucocorticoids were the standard of care for spinal cord trauma for years due to their anti-inflammatory properties yet were also associated with unwanted side effects. While the administration of glucocorticoids is controversial, immunomodulatory strategies that limit inflammatory responses provide the potential therapeutic approaches to promote functional regeneration following SCI. However, monitoring adherence to imaging criteria can be challenging and there are few established benchmarks for auditing BI-RADS 3 assignments. In this study, we explore some parameters that could serve as useful tools for quality control and clinical practice management.</p><p><strong>Materials and methods: </strong>This retrospective study covered a 4-year period (Jan 2014-Dec 2017) and included all women over 40 years who were recalled from a screening exam and had an initial assignment of BI-RADS 3 (probably benign) category after diagnostic workup. A follow-up period of 2 years following the assignment of BI-RADS 3 was used for quantitative quality control metrics.</p><p><strong>Results: </strong>Among 135,765 screening exams, 13,453 were recalled and 1,037 BI-RADS 3 cases met inclusion criteria. The follow-up rate at 24 months was 86.7%. The upgrade rate was 7.4% (77/1,037) [CI: 5.9-9.2%] and the PPV3 was 33.8% (26/77) [CI: 23.4-45.5%]. The cancer yield was 2.51% (26/1,037) [CI: 1.64-3.65%] and did not differ (p=0. 243) from the 2% probability of malignancy. The initial BI-RADS3 per screening exam and per recall from screening were 0.76% (1,037/135,765) [CI: 0.72-0.81%] and 7.7% (1,037/13,453) [CI: 7.26-8.17%], respectively.</p><p><strong>Conclusion: </strong>Regular audit of BIRADS 3 metrics has the potential to provide additional insights for clinical practice management. Prognosis Objectives: To clarify the features of lipomas that had been operated on in Kitasato University Hospital (KUH) and diagnosed by reviewing a series of cases. Method: All patients having undergone tumor removal between January 2000 and December 2021. Patients were identified through pathology department records. Results: Ages ranged from 2 to 80 years, with an average age of 47.1 years. Lipomas were benign in 38 cases and malignant in 2 cases. All 38 lipomas required only surgery. In cases of lipoma, 3 cases developed complications after surgery. In the cases of lipoma, although postoperative results were varied and changing, there were no recurrences. Conclusion: Clinical files were reviewed for 40 patients. Lipomas were benign in 38 cases and malignant in 2 cases. Complications after surgery were seen in 3 cases. Materials and Methods: An observational study, conducted in two private rehabilitation hospitals located in Italy, was conducted to obtain real-world data concerning the integrated rehabilitative pathway of 118 patients. Patients’ data were analysed adopting three methodological approaches. the predictors of the dependent variable. statistical approaches, comparative-configurational Results: Results reported that older age (β=0.145, p=0.049), higher sportiness level (β=0.169, p=0.022), as well as the length of the integrated rehabilitative pathway (β=0.550, p-value=0.000) and the performance of threshold tests (β=0.389, p-value =0.000) are antecedents of higher management costs (Adjusted R²=0.703 and F=24.482). Summarizing QCA results, age, and the level of sportiness of patients, in combination with the compliance, play an important role in the emergence of the selected and analyzed management costs.
