The definitive diagnosis is made at laparotomy. Perforated small bowel obstruction secondary to heavy Infestation with Ascaris Lumbricoides brings a number of intraoperative challenges requiring correct intraoperative surgical management decisions. We present a case of a 17 year-old patient who was admitted with a diagnosis of small bowel obstruction who at laparotomy was found to have perforated gangrenous small bowel volvulus with heavy worm load visible through the bowel wall. Concomitant diagnosis in the same patient is extremely rare with only one case report. Herein, we described an extremely rare case of concomitant pseudoaneurysm of mitral-aortic intervalvular fibrosa and aneurysm of left sinus of Valsalva in 21-year-old black man admitted with new-onset of heart failure (NYHA class III) diagnosed by transesophageal echocardiography. Despite medical therapy optimization for heart failure, the patient died waiting for cardiac surgery. Behfar","doi":"10.25149/JCRM.V8I1.165","DOIUrl":"https://doi.org/10.25149/JCRM.V8I1.165","url":null,"abstract":"Percutaneous left ventricular assist devices are approved for cardiogenic shock, but there has been increasing interest in utilizing these devices for hemodynamic support as bridge to durable mechanical circulatory support (MCS). A 63 year-old male with severe ischemic cardiomyopathy was evaluated for MCS. Given severe MR at baseline, it was felt that the Impella 5.0 device would offer the best hemodynamic optimization. Transesophageal echocardiography (TEE) showed severely enlarged left ventricle (78mm) and severe MR (effective regurgitant orifice (ERO) of 0.65cm2 and regurgitant volume (RV) of 94ml. After introduction of an Impella with TEE demonstrating correct placement, mean wedge pressure fell to 11 mmHg, lacking prominent V-waves. The severity of MR decreased to mild-moderate (ERO 0.17cm2 and RV 30 ml). He underwent LVAD implant after five days of support. Of these, most are benign. One such non-neoplastic lesion is Large Solitary Luteinized Follicular Cyst of Pregnancy and Puerperium (LSLFCPP). They are a rare lesion with unknown pathogenesis. They grow rapidly to very large sizes, causing symptoms. They are hormonally inactive. Maternal and fetal outcome is usually unaffected. \u0000Objective: we present a case of a Large Solitary Luteinized Follicular Cyst of Pregnancy and Puerperium encountered incidentally during the management of ruptured ectopic pregnancy. A 25 year old G2A1 with 7 weeks 3 days gestational age by dates presented to the emergency department with the chief complaints of sudden onset of pain in the lower abdomen accompanied by nausea and giddiness since 12 hours. What sets our case apart from rest in literature is it’s diagnosis very early in pregnancy. Also, it is, to our knowledge, the only case reported in literature, in an ectopic pregnancy. \u0000Conclusion: LSLFCPPis a rare, yet important entity to be kept in mind when evaluating patients with large ovarian cysts in pregnancy. Interstitial lung disease is a rare side effect of bicalutamide. Centres in the United States of America and Japan have previously reported bicalutamide-induced interstitial lung disease. This has not been reported in the United Kingdom. \u0000An 86-year-old Caucasian man with a history of advanced prostate cancer, on oral bicalutamide, presented to the emergency department with sudden onset dyspnoea and chest tightness. \u0000Bicalutamide-induced eosinophilic pneumonitis co-existing with community acquired pneumonia, and left ventricular systolic dysfunction were suspected, based on physical examination findings and investigation results that include blood tests, chest radiographs, echocardiogram, in addition to computed tomography pulmonary angiogram performed to rule out pulmonary embolism. \u0000There was a marked clinical, biochemical and radiological improvements after discontinuation of bicalutamide, initiation of a high-dose steroid, intravenous antibiotics, and diuretic. Prostate cancer retains a high incidence in the Western population. All of our patients were alive at a mean follow-up of 224,5 months (range 204-246). A gross total resection was accomplished in all cases. The development of a pleural effusion after transplantation requires further workup to determine if the etiology is malignant. We report a caseof a patient with recurrent left-sided pleural effusion without a definitive diagnosis despite multiple thoracentesis. Positron emission tomography–computed tomography (PET-CT) was performed that showed nodular pleural lesions in the left hemithoraxwith low level fluorodeoxyglucose (FDG) uptake not amenable to CT guided biopsy. Pleuroscopy allowed for direct visualization of the nodules and a diagnosis of non-Hodgkin lymphoma was obtained with forceps and cryoprobe biopsy. This unusual phenomenon may be secondary to various disease processes with a correct diagnosis seldom made preoperatively. \u0000Case presentation: The patient was a 64-year-old man with a medical history of distal gastrectomy for gastric cancer over 30 years ago. We diagnosed him with internal volvulus by several examinations and decided to perform emergency laparotomy. His abdominal pain improved markedly after inserting a long intestinal tube. At laparotomy, there was a large amount of chylous ascites in the abdomen. Despite an edematous small intestine with prominent lymphatic vessels, the cause of the volvulus was unclear, and there was also no evidence of ischemic change in the small intestine. She hadnever had hematuria or renal colic and urgently was admitted in a serious condition with large palpable abdominal mass. Thediagnostic workup, differential diagnosis, surgical management, medical follow-up and a review of the literature presented.Persistent back pain and weakness for several years were caused by nephrolithiasis and its late and rare complications likeRRL. Only one of the acute complications of nephrolithiasis as perinephric abscess attracted the attention of urologists andmade it possible to set up the correct diagnosis. Surgical management of the hephrolithiasis complications (urgent drainage ofperinephric abscess and delayed nephrectomy [because nonfunctioning kidney with multiple stones as a nidus of infectionsand RRL] after stabilization of patient’s condition was performed.Conclusion: Occasionally urolithiasis may mimic signs of spine disease, so in patients with persistent back pain which did notrespond to conventional therapy of this pathology should be excluded the hephrolithiasis.
