Seven heart transplantations were performed with a median blood component use of 105 units, which consisted of 32 (range, 14-41) units of red cell component (MAP), 34 (range, 10-56) units of fresh-frozen plasma (FFP), and 39 (range, 0-95) units of platelet concentrates (PC). Five lung transplantations were done with a median blood component use of 51 units, which consisted of 18 (range, 4-41) units of MAP, 17 (range, 0-44) units of FFP, and 16 (range, 0-35) units of PC, and 3 pancreas-kidney transplantations were done with a median use of 8 units, which consisted of 5 (range, 4-8) units of MAP and 3 units of FFP. As far as possible, CMV-negative blood components were supplied irrespective of CMV status of donor or recipient. To study the growth of this organism, 4 RBCs were inoculated with 1 or 10 colony forming units (cfu) /bag of Serratia liquefaciens (each concentration, 11=2) and stored at 5.6C . Each of the inoculated RBCs showed an apparent darkening in color on day 10 (10 cfu/bag) or 11 (1 cfu/bag). At that time, the number of Serratia liquefaciens had increased to 8.3 X 10 and 2.7 X 108 cfu/m 1, respectively. We then used a chroma meter to evaluate the change in RBC color more objectively and also photographed the samples for the three-week storage period. Supernatant Hb levels increased rapidly after day 7 and were 14.3 g/dl on day 10 (1 cfu/bag). Endotoxin was detected after day 9. These findings indicate that Serratia liquefaciens showed strong growth in RBCs stored at 5.6t, and that this proliferation of bacteria was associated with an increase in supernatant Hb and endotoxin level followed by darkening in color of RBCs. About 15,000 people register annually, the total number of donors was more than 210,000 and that of potential donors was 168,000 at the end of March 2003 (Fig. 1). However, this number is only about one-half of our goal of 300,000. The first unrelated bone marrow transplantation through the JMDP was carried out in the year after donor registration was initiated. The number of transplantations is growing yearly, 758 patients had transplantations in 2002. The total number was 4,752 at the end of March 2003 (Fig. 2). This number includes 10 cases transplanted with the help of the Korean Donor Bank. These figures are updated every three months and can be viewed on the JMDP web site. In 1996, a study group of the Japanese Ministry of Health and Welfare reported that HLA allele matching, particularly HLA-A and-B alleles matching, is important for successful BMT. The program has begun from 1987. Fifth grader students were surveyed to determine the effectiveness of this program at the end of the school year. Most students responded that they had improved their understanding of blood transfusion; in particular, they realized that blood products are obtained through the combined big efforts of volunteer donors and blood center workers. They also recognized that, as future doctors, they must minimize the use of blood products to avoid possible adverse side effects and also to ensure self-sufficiency of blood products in Japan. This problem is more serious in the management of patients with hematological malignancies or other types of bone marrow failure. Here we report a case of acute myeloid leukemia with IgA deficiency who underwent intensive chemotherapy and allogeneic bone marrow transplantation. To avoid the production of anti-IgA antibody, platelet concentrates were collected from donors who were IgA-deficient. Additional support with platelets and RBC components from donors who had circulating IgA was provided using products washed and replaced with 0.9% normal saline. Complete remission was achieved after a single course of remission induction therapy. After one course of consolidation chemotherapy, allogeneic bone marrow transplantation from an HLA-matched sibling was performed. No anaphylactic reaction was observed after any transfusion. No circulating anti-IgA antibodies were detectable after the full recovery of hematopoiesis in this patient. These findings indicate that allogeneic blood component products collected from donors with IgA deficiency are useful in myeloablative therapy in patients with hematological disorders. By washing with normal saline, products from donors with normal IgA levels are also acceptable. Further studies to elucidate the clinical usefulness of precise genomic and expressional analysis of IgA gene are required to ensure the safety of transfusion medicine in patients with IgA deficiency. However, it has become clear that prestorage leukocyte reduction has greater efficiency than poststorage, and prestorage leukocyte reduction is now being considered for implementation. We developed a prestorage leukocyte reduction system that contains CPD anticoagulant solution (PLD-C system). In this study, the performance of the PLD-C system was examined with regard to the leukocyte filtration efficacy and the characteristics of blood during storage by comparison with a pre-storage leukocyte reduction system which uses ACD-A solution (PLD-A system) and blood components collected using CPD solution and prepared by the buffy-coat depleted method (BCPD).It was confirmed that the leukocyte reduction ratio of the PLD-C system (more than-4log) was higher than that of the BCPD (less than-0.4log). Platelet depletion rates of the PLD-C group (average more than 99%) was also higher than in the BCPD group, but the red cell recovery ratio was the same between groups (average 88%). Prestorage filtration showed no adverse effects such as hemolysis, whereas various good effects were observed, such as inhibition of macroaggregate formation and hemolysis during storage. Collecting blood into CPD solution improved red cell quality during storage and increased plasma volume compared to the ACD-A solution.It was confirmed that the PLD-C system has advantages in the production of blood components over the present system. The efficacy of HbV has been demonstrated in the transfusion of HbV into rodent models of hemorrhagic shock. It is important to evaluate the compatibility of HbV with human blood cells. We examined the effects of HbV on human platelet activation in vitro by estimating the platelet release reaction and expression of platelet surface activation markers in the presence or absence of agonists. HbV concentration in the reaction volume was prepared at 20% or 40%. Preincubation of platelet-rich plasma (PRP) with HbV had no adverse effects on RANTES or serotonin release from platelets. Preincubation of whole blood with HbV also had no effects on exposure of P-selectin on platelets. However, binding of PAC-1, a monoclonal antibody that detects the activation-dependent conformational change of αIIbβ3 to platelets, was amplified by preincubation of whole blood with HbV in the presence of relatively low concentration of ADP. These results suggest that HbV enhances the binding of PAC-1 to platelets. At regular (5ml/min) and high (20ml/min) flow rates of infusion, red blood cells were filtered with a small delay. Filtered blood cells showed less damaged with regard to red cell morphology. A negligible increase in supernatant hemoglobin concentration was detected in filtered blood products, but no significant increase in supernatant potassium concentration was observed. The number of residual white blood cells after filtration was significantly reduced even at the forced high flow rate of infusion. In vitro analysis showed that Ec oliwas the cause of the acquired B phenomenon. The level of intensity of both Tk and acquired B antigen changed simultaneously and both disappeared at the same time. Both antigens were also abolished by acetylation of the patient’ s red blood cells. These findings indicate that the Tk polyagglutination was closely related to the acquired B antigen. H activity decreased during the course of hospitalization. From the results of acetylation of the patient’ s red blood cells the decrease in H activity was explained by the fact that acquired B‑and Tk‑expressing red blood cells showed stronger than H activity of A1 red blood cells. The antigenic modification appeared prior to the onset of Ec olisepticaemia. Sixteen exchange transfusions were performed upon 10 infants whose birth weights ranged from 512g to 3610g. The incidence of exchange transfusion was 0.44%. The indication for each exchange transfusion was hyperbilirubinemia (eight instances), sepsis with DIC (four instances), shock with massive hemorrhage (three instances) and hydrops fetalis. Hyperkalemia was a complication in one case. Adequate blood was difficult to get in emergency situations.Exchange transfusion is one of the essential techniques in the neonatal care, although it may involve some complications including hyperkalemia, bleeding, hypocalcemia, hypoglycemia, graftversus-host disease, infections, etc., especially in very low birth weight infants. In our nursery, the incidence of exchange transfusion was very low in comparison with that of previous reports. This may represent a difference in the indication of this therapy.We propose that we should discuss the appropriate indications and safe procedures concerning exchange transfusion.
