Adam","doi":"10.24170/18-1-4774","DOIUrl":"https://doi.org/10.24170/18-1-4774","url":null,"abstract":"Background: We sought to provide a descriptive report of inpatient admissions for cerebrovascular disease (CVD) at a tertiary hospital in KwaZulu-Natal, South Africa. \u0000Methods: We conducted a retrospective, descriptive study involving 1 017 patients admitted to the Inkosi Albert Luthuli Central Hospital for treatment of CVD during 2005 - 2015. Patients were identified using the hospital’s electronic administrative system. Demographics, inpatient length of stay, surgical procedures, and survival status at discharge were also obtained for each patient. Descriptive statistics were used to analyse the data. \u0000Results: The median age of CVD patients was 50.0 years, and 50.4% were male. The median inpatient length of stay was 13.0 days. Surgery was performed in 38.8% of patients. The degree of anticoagulation is monitored by evaluating the international normalised ratio (INR); however, the quality of anticoagulation, determined by the time in therapeutic range (TTR), is less often evaluated. TTR has significant clinical implications in patient outcomes. \u0000Objectives: We sought to identify the indications for anticoagulation and determine its quality via the TTR at a single centre, community-based and district level hospital in the setting of usual care. We documented the prevalence of thrombo-embolic and haemorrhagic adverse events and we also collected data on factors that may contribute to a poor TTR or increased risk of adverse events. \u0000Methods: We conducted a retrospective, descriptive and observational study with chart audits evaluating the anticoagulation indication and control for the preceding 1 year for each patient. Descriptive statistics included mean and standard deviation for quantitative data and frequencies for categorical data. Chi-square tests were used to analyse comparisons of categorical data and the student’s t-test for continuous variables. Two-tailed p-values less than 0.05 were considered significant. \u0000Results: TTR was poor for patients with AF and PHV (44.5% and 13.7% respectively). We identified older age, less frequent testing and high target ranges as significant factors associated with poorer outcomes. Nurses are often the first responders to in-hospital arrest and are expected to initiate cardiopulmonary resuscitation (CPR). It has been identified in the literature that nurses lack the knowledge and the skills of resuscitation. The aim of the study was to determine the CPR skills profile and knowledge among nurses in selected areas at Chris Hani Baragwanath Academic Hospital. \u0000Materials and methods: The study was descriptive, prospective and contextual. A self-administered questionnaire, based on the 2010 American Heart Association CPR Guidelines, was used to determine the \u0000nurses CPR knowledge. A minimum score of ≥80% was regarded adequate. \u0000Results: Total of 133 nurses were recruited; 61 in theatre, 40 in ICU and 32 from the surgical wards. The nurses’ median score (range) for theoretical knowledge was 21% (0% - 79%). No participant achieved ≥80%. No correlation was found between nurses’ knowledge and years of experience (r=0.036, p=0.697). Comparison between knowledge scores and area of work (p=0.060) and accreditation status (p=0.444) were not statistically significant. \u0000Conclusion: The knowledge of CPR was found to be poor among nurses and has shown the need for ongoing CPR training to ensure effective resuscitation of patients.
