Bloodless angioscopy during instructional PCI, which is possible in the Visible Heart Laboratories (VHL), can provide valuable insights into dissection morphology and creation.</p><p><strong>Aims: </strong>We aimed to investigate periprocedural coronary artery dissections in porcine hearts during educational bifurcation stenting sessions in the VHL.</p><p><strong>Methods: </strong>Between July 2019 and October 2023, expert operators performed bifurcation PCI on the Visible Heart® apparatus (bloodless beating heart system). Interventional strategies were at the operator's discretion, though intentional stent oversizing is necessary to prevent stent migration, which predisposes to dissections. Angioscopy and Optical coherence tomography (OCT) footage was reviewed for dissections, which were categorized by morphology, creation mechanism and progression in extent.</p><p><strong>Results: </strong>Of 119 bifurcation PCIs, 53 showed coronary artery dissections. Two main dissection morphologies emerged: Longitudinal (in line with vessel, n=14) and transverse dissections (n=34). Transverse tears, caused by deep angioscope/guide catheter intubation (n=29), attempts to cross balloons against resistance (n=3), but also by kissing balloon inflations (n=1) and geographical miss (n=1) of post-dilatation, progressed in 80.2% of cases (118.8±79.4° to 195.1±116.7° lateral extension, p<0.001). Longitudinal coronary artery dissections predominantly resulted from first kissing balloon inflation during double kissing (DK) crush technique and did not progress relevantly (2.9mm [2.4 - 4.2] to 3.1mm[2.7 - 4.7], p=0.68).</p><p><strong>Conclusions: </strong>This is the first report demonstrating angioscopic images of coronary artery dissections. Temporal disease trends, treatment patterns, and subgroup differences by sex and age were assessed.</p><p><strong>Results: </strong>A total of 37,741 hospitalizations among 33,645 patients were analyzed [median age: 70 years, 66.4% male]. The most frequent primary discharge diagnoses were coronary artery disease (CAD) (40.8%), acute decompensated heart failure (HF) (20.4%), and arrhythmias (18.6%). Hospitalizations due to chronic coronary syndrome and hypertensive heart disease gradually declined, while those due to aortic stenosis, HF, pulmonary embolism, pericarditis, and non-ST-elevation acute coronary syndrome increased. The overall prevalence of HF with reduced ejection fraction, valvular heart disease (VHD), pulmonary embolism, and tachyarrhythmias rose over time. Sodium-glucose cotransporter-2 inhibitors, mineralocorticoid receptor antagonists, non-vitamin K antagonist oral anticoagulants, and statin-ezetimibe therapy showed increased use, while vitamin K antagonists and dual antiplatelet therapy declined. Percutaneous coronary interventions declined over time. Women had higher rates of VHD, arrhythmias, and pulmonary embolism; men had more CAD and cardiomyopathies. However, an increasing number of individuals are simultaneously suffering from multiple CMDs, namely cardiometabolic multimorbidity (CMM). The extent to which CMM increases the risk of HF remains unclear. We aimed to explore the additive association of the number of CMDs with the risk of HF and evaluate whether the genetic risk of HF would modify this association.</p><p><strong>Methods: </strong>We included 374,565 participants from the UK Biobank. The number of CMDs (including coronary heart disease, diabetes, stroke, and hypertension) and the polygenic risk score for HF (low, intermediate, and high) were assessed as exposures. The adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) for incident HF were calculated using the Cox proportional hazards model.</p><p><strong>Results: </strong>During a median follow-up of 14.22 years, there were 13,154 cases of incident HF. Compared with participants having no CMD, the adjusted HRs for those with 1, 2, and ≥3 CMDs were 1.66 (95% CI: 1.59-1.73), 2.93 (95% CI: 2.77-3.10), and 5.13 (95% CI: 4.73-5.56), respectively (P<sub>trend</sub> < 0.001). Synergistic interactions were observed between high genetic risk and CMDs. Participants with high genetic risk and ≥3 CMDs were estimated to have a 6.4-fold elevated risk of HF compared with those with a low genetic risk and no CMD.</p><p><strong>Conclusion: </strong>The number of CMDs is dose-dependently associated with incident HF. And there is a synergistic interaction between high genetic risk and CMDs. The evolution of transcatheter aortic valve replacement (TAVR) and its expanding indications for symptomatic patients with severe AS, coupled with the evolving understanding of the pathophysiology and natural history of AS, have heightened the focus on asymptomatic patients with severe AS. Although current clinical practice guidelines recommend aortic valve replacement (AVR) therapy in asymptomatic severe aortic stenosis (ASAS) only in specific clinical settings, recent studies have challenged traditional treatment paradigms, advocating for a more individualized strategy, particularly for patients exhibiting high-risk characteristics. In this review, we provide an in-depth analysis of ASAS, focusing on the intricacies of its clinical management, novel risk-stratification modalities, and predictors of symptom onset and disease progression. We also determine the role of echocardiography in assessing AS severity, highlighting inconsistencies in diagnostic criteria and the need for supplementary testing.
