The questions on this scale reflect the corresponding clusters ofPTSD symptoms according to the DSM-5 classification. Clinical-psychopathological andstatistical methods were also used.Using a screening test, it was established that the serviceman experienced severepsychological trauma related to the war, the memories of which are still «unrelenting».Using the NADS test, the patient was diagnosed with clinically expressed anxiety (14points) and subclinical depression (10 points). Interpreting the results of the study interms of clinically significant symptoms, in accordance with the PCL-5 scale, it wasconcluded that according to the total number of points and the sum of points calculatedby clusters of symptoms, the serviceman has post-traumatic stress disorder. The patientwas prescribed: psychopharmacotherapy (sedative antidepressant amitriptyline (50 mg),anxiolytic miaser (30 mg), anti-anxiety neuroleptic eglonil (50 mg)) and trauma- focusedcognitive- behavioral psychotherapy, which increased the patient’s resourcefulness. The diagnostic method of the clinical interview (MCI),clinical- psychopathological and statistical method were used among 30 (13 girls and 17boys) socially deprived adolescents of school age to detect mental maladjustment.The results. It has been established that psycho- emotional stress, neurotic changes andinstability of emotional reactions, especially in deprived teenagers, are caused by psychotraumatic situations (acute or chronic) and an unsatisfactory social and psychologicalclimate in the family, home, and educational institution.Strong emotional experiences of 62 % of girls (9 persons) were due to the breakup offriendships, a small number – 28 % (4 persons) due to punishment for failure at school andthe least due to the indifference of others 24 % (3 persons). Three girls noted 2 signs each.Among young people, emotional and stressful situations were caused in the largest partdue to the indifference of others – 72 % (12 people), a fairly high rate due to the breakupof friendly relations – 32 % (5 people) and the least was observed in cases of punishmentfor failure – 6 % (1 person). One young man noticed 2 signs in himself. Among thoseexamined, 70 % of teenagers were witnesses of psychological bullying, 27 % of the subjectsperiodically insulted their peers at school, and 1 boy (3 %) admitted that he sufferedbeatings from high school students. The difference is probable (Р<0.001) in comparisonwith indicators of mental and physical violence. No one described themselves as criminals.Conclusions. LPO and AOS indicators were studied at 24-26 and 34-36 weeksof pregnancy in 20 healthy women with a physiological course of pregnancy (controlgroup) and in 34 women with PE (main group). 20 of them- with moderate PE (groupI) and 14 – with severe PE (group II).The results. When studying the processes of POL-AOS in pregnant women of all groups,a reliable increase in POL was found, which was more significant in the main group(Р˂0.001). In response to the activation of LPO processes in the control group, the activationof the AOS components of blood protection occurred, as evidenced by an increase in thelevel of glutathione (Р˂0.02) and glutathione peroxidase (Р˂0.05). This contributes to theneutralization of POL products. In pregnant women of the I group, at 24-26 weeks, the levelof glutathione and glutathione- peroxidase increased (Р˂0.02 and Р˂0.01, respectively), andat 34-36 weeks, these indicators did not differ from the control (Р>0.05). In women of theII group, against a background of an increase in the level of lipoperoxidation products, theindicator of glutathione peroxidase did not increase reliably (Р>0.05). The level of glutathionein 24-26 weeks also did not increase (Р>0.05), and in 34-36 – significantly decreased.Conclusions. In women with moderate PE at 34-36 weeks of pregnancy, there is anantioxidant deficiency and a decrease in compensatory and adaptive mechanisms.Exhaustion of the body’s antioxidant defense system was found in pregnant women withsevere PE. Violation of AOS function against a background of hyperproduction of lipidhydroperoxides can lead to oxidative stress, disruption of compensatory and adaptivemechanisms and the development of pathological conditions in the mother and fetus.Preclinical diagnosis and correction of disorders of POL-AOS processes will give anopportunity to prevent the development of severe PE.","PeriodicalId":475378,"journal":{"name":"Клінічна та експериментальна патологія","volume":"3 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136010872","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The relationship between the number of CD4 lymphocytesand the presence of markers of pathogens’ active replication of co-infections, caused byherpesviruses (cytomegalovirus – CMV, herpes simplex viruses of types 1 and 2 – HSV½,Epstein- Barr virus – EBV), and hepatitis B, C viruses in the plasma of HIV-infectedpatients, was considered.The purpose of the study – to determine the spectrum of viral co-infections in patientswith HIV infection and to find out the impact of these infections on the developmentof immunodeficiency based on the number of CD4 lymphocytes. Materials and methods. 146 patients with HIV infection, who were under outpatientobservation during 2020-2022 in the regional center of prevention and fight against AIDS inChernivtsi, were examined. When establishing the diagnosis, clinical and epidemiologicaldata and the results of laboratory tests, in particular: serological and immunological, weretaken into account. The level of CD4+ T-lymphocytes was examined after the disappearanceof the symptoms of the accompanying acute infectious disease (at least after 4 weeks).The results. The significance of the clinical manifestations and the nature of the course ofherpes simplex and shingles against a background of HIV infection depended on the degree ofreduction in the number of CD4+ lymphocytes. In most of the examined (57.0 %), the numberof CD4+ lymphocytes was less than 500 cells in 1 mm3 of blood, which contributed to theactivation of herpes simplex and shingles. A careful analysis of the frequency of detection ofserological markers of viral co-infections, depending on the route of HIV infection, showedthat hepatitis B virus markers were significantly more often detected in persons with a sexualroute of infection, and hepatitis C and mixed hepatitis virus markers – in those personsinfected with HIV, most likely, due to parenteral administration of narcotic substances.Conclusions. 1. Analysis of the spectrum of co-infections of viral genesis in HIV-infectedpersons demonstrated a wide distribution of parenteral viral hepatitis and a high levelof activation of chronic infections caused by HSV½, CMV and EBV. Markers of viralhepatitis B and C were detected in 56.8 % of patients with HIV infection. Activationof chronic infections, caused by CMV, HSV½ and EBV, was observed in 32.2 % of thesubjects. 2. Serological markers of viral hepatitis C and mixed hepatitis (HS + HBV)are most often detected in persons with parenteral HIV infection, and markers of viralhepatitis B – mostly in persons with sexual infection of HIV. 3. Experimental investigations were carried out under conditionsof the ballistic route of the National Scientific Center «Hon. Prof. M. S. Bokarius ForensicScience Institute». To determine the velocities of the fired bullets, the optoelectronicmeasuring complexes IBX-731.4 were used. Ballistic plasticine «Beschussmasse,6287156» was used as a simulator of human body tissues. The experimental shooting wascarried out with a TS.M308 model .308 «Winchester» sniper rifle. For the experimentalshooting 7.62 × 51 mm (.308 «Winchester») cartridges were used, which were filled with«SP» and «HPBT» bullets.Results. In the course of experimental shooting, the peculiarities of the formation ofdamage channels caused by 7.62 mm «SP» and «HPBT» bullets were determined. Basedon the results of experimental studies, it has been found that the bullets of these types ofthe same caliber and practically the same mass are able to cause different amounts ofbodily injury, which is primarily due to the peculiarities of the design of the bullets. SPtype bullets with a bare core cause more severe bodily injuries, they are less subject tofragmentation and in most cases retain a relatively heavy bottom part. As a consequence,such bullets are able to cause longer wound channels than other types of expansivebullets. HPBT-type bullets can also cause sufficiently large amounts of bodily injury,but because of their design, they are more prone to fragmentation when hitting thehuman body. In the course of the research it has been established that there are severalperipheral (secondary) channels in the area of the main wound channel. They are formeddue to destruction of the bullet and movement of its separate fragments away from themain direction, which leads to complication of diagnostics and further treatment of suchgunshot wounds.Conclusions. The use of 7.62 × 51 mm (.308 «Winchester») expansive «SP» and «HPBT»type bullets in the course of combat operations for firing modern sniper systems resultsto severe bodily injuries. Wounds caused by such bullets are characterized by significantin volume zones of direct traumatic necrosis (contusion), which significantly extend deepinto the tissues and structures of the human body and form even larger in size zonesof conditional necrosis (contusion) and morpho- functional changes. The formationnature of both the main wound channel and the peripheral wound channels, caused byfragments of bullet structural elements, depends on the velocity, design features andposition (projection) of the bullet at the moment of impact. The main group consisted of 12 patients who hada complete eventration (grade II-IV). The comparison group was formed by 16 patients whohad subcutaneous eventration (I degree). The collection of biological material was carriedout during the 8th day of the early postoperative period, by bacteriological examinationof the wound exudate, during dressings. Species composition of microorganisms, theirpopulation level, the coefficient of constancy, the frequency of species detection, thecoefficient of significance, the coefficient of quantitative dominance was studied.Results. When eventration II-IV degrees occurs, against a background of the oncologicalprocess, a probable predominance of the number of strains of E. coli, S. aureus and m/o ofthe genus Proteus is noted, and the number of strains of E. coli is likely to prevail during allperiods of observation. During the development of II-IV degrees of eventration, due to theoncological process, in the exudate of the laparotomy wound, there is a probable predominanceof the population level of E. coli, S. aureus and m/o of the genus Proteus. The dominantmicroorganism, according to the determined coefficient of quantitative dominance, at alldegrees of eventration, throughout the entire period of the study, is E. coli, and Enterococcusfaecalis is in the second place. At the same time, at all degrees of eventration, high numbers ofthe coefficient of quantitative dominance in S. aureus are noted on the 6-8th day of the study. Conclusions. With the development of postoperative eventration of II-IV degrees, inpatients with malignant neoplasms of the abdominal cavity, a probable increase in thenumber of strains sown and the population level of microorganisms in the laparotomywound exudate with E. coli, S. aureus and microorganisms of the genus Proteus is noted.In patients with malignant neoplasms of the abdominal cavity, with the occurrence ofII-IV degrees of postoperative eventuration, the dominant microorganisms of the woundexudate are E. coli and Enterococcus faecalis and microorganisms of the genus Proteus.","PeriodicalId":475378,"journal":{"name":"Клінічна та експериментальна патологія","volume":"44 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136010874","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
