Lysokon","doi":"10.26693/jmbs08.01.008","DOIUrl":"https://doi.org/10.26693/jmbs08.01.008","url":null,"abstract":"The purpose of the work was to study the etiology and main etiopathological factors of the development of aggressive forms of periodontitis based on the analysis of additional pathogenetic aspects of mineral metabolism disorders according to modern literature. Materials and methods. The bibliosemantic method was used to clarify the state of the problem, study the analysis of the results of previous scientific research based on literature sources and electronic resources. Results. Periodontitis is the most widespread disease among all periodontal pathologies (chronic generalized periodontitis occurs in 90% of cases). Until recently, aggressive forms of periodontitis were not given due attention, since according to epidemiological studies by S. Renvert and M. Wikstom (1996), in the general structure of periodontal diseases, aggressive forms of periodontitis occur with a frequency of 5–10%. To understand the mechanisms of the development of aggressive forms of periodontitis, attention is paid to their development in children and adolescents. Clinical and radiological examinations of children and adolescents showed different frequency of aggressive forms of periodontitis (localized juvenile periodontitis and generalized juvenile periodontitis): in the USA – 2.27%, in Denmark – 0.1%, in Finland – 0.1%, in Nigeria – 0.8%, in Brazil – 0.3%, in England – 0.17%, in Chile – 0.32%. Studies have shown that aggressive forms of periodontitis are equally common in children and adolescents, regardless of gender. At the same time, localized juvenile periodontitis is 3 times more common in boys, not the same for girls. A new and rather promising vector of periodontology is the study of the role of juvenile changes in the bone skeleton on the development and course of periodontal diseases. The development of mineral metabolism disorders in aggressive forms of periodontitis in different age groups requires further study, while the data obtained by molecular geneticists become relevant for understanding the possible ways of research. Considering the fact that the jaw bones are a component of the skeleton, the strength of which can be affected by various exogenous and endogenous factors, in particular genetic ones, the main direction of searching for candidate sensitivity genes is conducted among the markers of bone metabolism disorders. Of many candidate genes that determine bone mineral density, the most well-known is the vitamin D receptor gene. Conclusion. Materials and methods. 62 children aged 6 years with fissure caries of the first permanent molars (main group) and 31 children without signs of carious lesions (control group) were examined. The study of the crystal formation of oral fluid was carried out according to the method of Leus P. A. (1977). The mineralizing potential of oral fluid was expressed as an average score depending on the types of crystals detected. Results and discussion. The analysis of the results of the study showed that 6 months after the start of preventive measures in the oral fluid of the children of the main group, a slight increase in type I crystals was noted in comparison with the initial data (40.18 ± 4.65% of cases, p>0.5), at the same time, the number of children, in whom type III crystals were observed, decreased in the main group (16.45 ± 2.32%, p>0.5), and increased in the control group (19.32 ± 2.47% of cases, p>0.5) in comparison with the initial data. After two years of observation, in the children of the main group, type I crystals were most often detected in the oral fluid (48.41 ± 4.23% of cases), in the children of the control group, on the contrary, the number of children with this type of crystals decreased and was almost one and a half times less than among children of the main group (p<0.05). On the other hand, the number of children in the control group, in whom type III crystals were detected, was two times higher than among children in the main group (22.34 ± 2.87% and 11.51 ± 2.56% of cases, respectively, p<0.05). Conclusion. It was established that in children who received a complex of preventive measures, 6 months after the implementation of preventive measures, the mineralizing potential of oral fluid was, on average, 2.73 ± 0.07 points, which is slightly higher compared to the fissure detected at the beginning of caries prevention (2.57 ± 0.12 points respectively, p>0.05). In the children of the control group, the mineralizing potential of oral fluid changed less noticeably (from 2.64 ± 0.11 points at the beginning of observation to 2.66 ± 0.11 points, p>0.5). After two years of observation, it was found that in the main group the mineralizing potential of oral fluid continued to increase, while in the children of the control group it decreased and reached almost the initial level (2.65 ± 0.07 points in the second year of observation and 2.64 ± 0.08 points at the beginning, p>0.05). Materials and methods. Analysis, generalization and systematization of scientific and methodological literature were used in the study. Results and discussion. Integration significantly changes the content and structure of modern scientific knowledge, intellectual and conceptual capabilities of certain sciences. It is the most important means of achieving the unity of knowledge in the content, structural, scientific, organizational, and pedagogical aspects. Integration in the fundamental fields arose on the background of differentiation of sciences and detailing of specialties, the growing amount of knowledge and requirements for it in each individual field. Integration between academic disciplines is aimed at deepening the interconnections between subjects and is a way to improve it. The interdisciplinary integration of basic disciplines in higher medical institutions provides for a unified approach of the teaching staff of different disciplines to solving common educational problems based on the generalization of knowledge by students. Histology, cytology, embryology and human anatomy as disciplines that study the structure and morphological variations of the human body are perceived from the viewpoint of modern science as the basis of medicine, physiology and pathology, which has been gradually formed since ancient times and up to the current day. Such a close relation between the micro- and macro-structure of the body is consistent and logical in the study of two fundamental disciplines, it becomes possible to transfer theoretical knowledge, practical skills and abilities from one discipline to another. Creating interdisciplinary connections contributes to deeper learning and repetition of previously studied material, and properly organized integration plays both an educational and an upbringing role. The interdisciplinary integration of two fundamental disciplines such as Histology, Cytology and Embryology with Human Anatomy is a necessary component of the training of a future doctor. The use of such interdisciplinary technologies for training a doctor allows to raise him/her to a qualitatively new level of clinical thinking, capable of solving the problems of medical practice in a comprehensive manner based on the wide integration of information from various disciplines. Materials and methods. The following effective and objective methods of assessing the functional state of patients after ischemic stroke were used in the study: analysis of special scientific and methodical literature, clinical and instrumental research methods (survey, Berg balance scale, VAS, MMT Lovett, Cross-out test stars, Stand and Walk Test, 10-meter test, Rivermead Index, Modified Rankin Scale, Montreal Cognitive Dysfunction Scale, Hospital Depression and Anxiety Scale), methods of mathematical statistics. In order to evaluate the effectiveness of the developed program of physical therapy for patients with ischemic stroke, a study was conducted of 20 patients, aged 45-65, at the inpatient rehabilitation department. The proposed method contributes to the restoration of balance, coordination and gait speed, muscle strength, activity in everyday life, level of self-care, cognitive abilities, reduces pain syndrome, severity of neglect, level of depression and anxiety. The presented materials can be used in the process of activity of specialized medical, rehabilitation and health facilities, medical centers, as well as for practical study by students of higher educational institutions specializing in “Physical therapy, occupational therapy”. The study was conducted from November 2021 to November 2022 in the department of physical and medical rehabilitation of Kyiv City Clinical Hospital No. 6, which provides scheduled specialized medical care to patients with neurological and orthopedic pathologies. Results and discussion. 20 patients were evaluated, 10 patients were assigned to the main group and 10 – to the control group. The initial indicators of the studied parameters in patients of both groups did not differ statistically significantly (p>0.05). The age group of patients is from 45 to 65 years old. The effectiveness of rehabilitation largely depends on the individual program developed by a professional. The basis of the programs is an individual approach and the patient's return to the most fulfilling life and adaptation in society. Positive results of rehabilitation are possible only with a comprehensive approach to restoring lost functions. Conclusion. Materials and methods. An anonymous online questionnaire and psychodiagnostic examination of 195 students majoring in “Medicine”, “Dentistry” and “Medical Psychology” were conducted, using the Beck Depression Inventory and H. Eysenck's scale of self-assessment of mental states. Results and discussion. It has been established that psychological problems related to distance learning are more common among senior students, and are most common among students majoring in “Medical Psychology”, less common among students majoring in “Medicine” and the least – among students studying dentistry. It was found that the most relevant negative psychological consequences of distance learning are an increase in anxiety: in 21.9% of junior students and in 46.6% of senior students studying in the specialty “Medical Psychology”, in 25.7% and 38.2 % – in the specialty “Medicine”, in 18.2% and 22.6% – in the specialty “Dentistry”; mood deterioration: respectively in 62.65% and 60.0%, in 17.2% and 29.4%, in 9.1% and 22.6%; irritability: 68.8% and 53.3%, 22.9% and 35.3%, 15.1% and 22.6%, respectively; deterioration of communication with others: 46.9% and 46.7%, 22.9% and 17.7%, 21.2% and 25.9%, respectively. Higher levels of depression were found in students majoring in “Medical Psychology”: 7.94 ± 4.40 points and 8.80 ± 4.99 points against 3.77 ± 3.08 points and 4.38 ± 3.06 points for students studying medicine, and 2.70 ± 2.42 points and 3.35 ± 2.42 points for students studying dentistry; anxiety: 6.72 ± 6.59 points and 11.23 ± 4.70 points, 7.37 ± 5.31 points and 9.09 ± 4.83 points, respectively; 6.21 ± 4.87 points and 7.61 ± 4.94 points; frustrations: 8.38 ± 5.17 points and 10.67 ± 5.94 points, respectively; 2.86 ± 3.65 points and 4.59 ± 5.10 points; 1.82 ± 2.49 points and 2.74 ± 3.12 points; and rigidity: 7.53 ± 3.61 points and 8.07 ± 4.24 points, respectively; 4.86 ± 4.56 points and 6.53 ± 4.63 points; 6.55 ± 4.80 points and 6.58 ± 4.33 points. The highest levels of aggressiveness were found in students studying dentistry: 9.36 ± 2.73 points and 8.90 ± 4.07 points against 8.74 ± 3.86 points and 7.97 ± 3.55 points for students studying in the specialty “Medicine” and 5.03 ± 3.86 points and 4.80 ± 3.49 points in the specialty “Medical Psychology”. Among students studying medicine and dentistry, the vast majority showed no signs of depression, while among medical psychology students, 6.3% of examined juniors and 13.3% of seniors showed signs of moderate depression. Signs of severe anxiety were found in 5.9% of senior students majoring in Medicine, in 9.4% of junior students and in 16.7% of senior students of “Medical Psychology”, signs of severe frustration were found in 8.8% of senior students majoring in “Medicine” and 12.5% and 20.2% of students majoring in “Medical Psychology”, respectively. Nogas","doi":"10.26693/jmbs08.01.208","DOIUrl":"https://doi.org/10.26693/jmbs08.01.208","url":null,"abstract":"The purpose of the study was to assess functional disorders of the upper limbs in patients with rheumatoid arthritis using the Boston Carpal Tunnel Questionnaire. Materials and methods. To assess the functional status of the affected upper limbs of patients with rheumatoid arthritis, the Boston Carpal Tunnel Questionnaire was used, which consists of two scales: the Symptom Severity Scale and the Function Status Scale, which were filled out by the patients themselves. The study was conducted on the basis of rheumatology department of the Municipal Enterprise “Yuriy Semeniuk Rivne Regional Clinical Hospital” in Rivne during the period from 2016 to 2018. 216 patients took part in the examination, of which 178 (82%) were women and 38 (18%) were men. The patients’ age is from 25 to 59 years old (average age is 46 ± 0.24). The inclusion criteria were: the presence of an established diagnosis of rheumatoid arthritis based on the ACR/EULAR 2010 criteria, and in accordance with the Order of the Ministry of Health of Ukraine dated April 11, 2014 No. 263 “Unified clinical protocol of primary, secondary, of tertiary medical care and medical rehabilitation of patients with rheumatoid arthritis”, the presence of informed consent of patients to participate in the study. Patients' rights were respected in accordance with the Declaration of Helsinki “Ethical Principles of Medical Research Involving Human Subjects” developed by the World Medical Association, “Universal Declaration of Bioethics and Human Rights (UNESCO)”. Results and discussion. During the initial examination, it was found that the majority of patients had moderate (20.3%) and severe pain (16.6%) in the affected upper limbs (hands / wrists), which in the last two weeks had caused the patients to wake up at night: once – 10.6% of patients, 2–3 times –15.7% of patients. During the day, 20.3% of patients complained of moderate pain in a hand / wrist, and 17.5% of people were bothered by severe pain. The majority of patients (15.7%) noted that the pain lasted from 10 to 60 minutes. As for functional disorders, the greatest difficulties for patients occurred when buttoning clothes – 45.3%, doing household chores – 39.8%, and opening a bottle – 36.1% of patients. Manifestation of local symptoms of damage to the carpal tunnel affects the general physical and psychological condition of the patient. The patient loses work capacity, concentration, and the quality of his / her life decreases. Conclusion. Materials and methods. 104 patients, aged 28–46 years, with diseases of periodontal tissues, who had somatic pathology, were involved in the study. The examined persons were divided into groups: 35 persons (group 1) with pathology of the hepatobiliary system, 37 persons (group 2) with pathology of the gastrointestinal tract, and 32 persons (group 3) with pathology of the cardiovascular system. The comparison group included 30 people with periodontal tissue diseases without somatic pathology. To determine the depth and prevalence of the inflammatory process of the gums the Svrakov’s number indicator was used. Oral hygiene was assessed according to the Silness-Loe and Stallard indices. The papillary-marginal-alveolar index (C. Parma, 1960) was determined to assess the severity of gum inflammation. Bleeding gums were assessed by the gum's papilla bleeding index according to Muhllemann. Statistical processing of the research results was carried out with a computer program for variational and statistical analysis of medical and biological research data “GraphPad Prism 5”. Results and discussion. Patients with diseases of periodontal tissues against the background of somatic pathology had the highest percentage of chronic generalized periodontitis of the II degree, in contrast patients of the comparison group had the highest percentage of chronic generalized periodontitis of the initial – I degree. Svrakov's number in patients with periodontal disease against the background of somatic pathology significantly (р ˂0.05) exceeded the indicators of patients of the comparison group. At the same time, there is no significant difference between the results of groups 1, 2 and 3. The highest values of the indexes of Silness-Loe and Stallard were recorded in patients with the cardiovascular system pathology, which significantly (p<0.05) exceeded the data in the comparison group, but did not significantly differ from the results of patients with pathology of hepatobiliary system and gastrointestinal tract. The papillary-marginal-alveolar index data were the highest in patients with hepatobiliary system pathology (64.25 ± 8.19%), which significantly (p<0.05) exceeded the results of the comparison group and in individuals of groups 2 and 3. The highest indicator of the bleeding index is 1.98 ± 0.22 points in patients of the 1st group, which significantly exceeds the indicators in the group of people without somatic pathology and in the group of patients with pathology of gastrointestinal tract and cardiovascular system. Conclusion. Materials and methods. The work uses such research methods as modeling of food deprivation according to a patented method, modeling a wound defect of rat’s skin, evaluating the healing of wound defects by taking pictures with a camera with a resolution of 12 megapixels and subsequent processing with a special computer program Universal desktop ruler, version 11, for determining the area, determination of integral indicators of total protein, albumin, glucose, urea in blood serum of rats by spectrophotometric methods (“Filicit-diagnostics”). Statistical processing of the obtained results was carried out using non-parametric methods of analysis (MANOVA-test) using Statistica 6.0 software. Differences at p<0.05 were considered significant. Results and discussion. As a result of the food crisis, the share of hungry people in the world increases every year, and in 2022 it reached 9.8% of the world population. Starvation directly and indirectly prevents the successful healing of wounds, which is also complicated by impaired liver function. Partial food deprivation was accompanied by a violation of metabolic processes in the body of rats: a decrease in body weight (about 30%, p<0.05), a probable decrease in the content of glucose (by 24.5%), total protein and albumin (by 16.1 and 26.2%, respectively, p<0.05) with a simultaneous increase in the content of indicators of nitrogen metabolism in the blood. In addition, nutritionally deficient animals had a 4.7-day delay in wound healing compared to the control group. The use of arginine glutamate, in food-deprived rats caused a reduction in the wound skin defect and was accompanied by a partial normalization of metabolic processes: the total protein content and the amount of blood albumin were higher than those in the model pathology group without correction by 10.2 and 12.9%, respectively (p<0.05), the glucose content increased (+12.2%); simultaneously blood urea content was lower by 25.2% (p<0.05). Conclusion. Materials and methods. The experimental part of the study was performed on 78 rabbits (Chinchilla breed): 15 animals – the control group and 63 animals – the experimental group (31 females and 32 males). After placement of implants, in the blood of the experimental animals, after 1, 3, 6 and 12 months, the activity of alkaline phosphatase was determined by a unified colorimetric method and the activity of tartrate-resistant acid phosphatase – by an immunoenzymatic method using the “Bone TRACP Assay” kit by Assay Pro (USA). Results and discussion. On the 12th month of the experimental research in the experimental animals of subgroup A (I), the activity of alkaline phosphatase in the blood was significantly lower compared to the data of intact animals, p<0.01 and 64.71% lower, compared to the initial data, p1<0.01. It was noted that after 12 months of observation in animals of subgroup B (I), the activity of alkaline phosphatase in the blood was equal to the data of intact animals, p>0.05, and was 72.06% lower than the initial values, p1<0.01. After 12 months of research, the animals of the subgroups, where no correction of simulated osteoporosis was performed, observed an intense increase in the activity of tartrate-resistant acid phosphatase in the blood. At the same time, in the subgroups where the medicinal treatment of osteoporotic phenomena was carried out, there was a tendency to decrease the activity of tartrate-resistant acid phosphatase in the blood, the data of which after 12 months of observation were equal to the values in intact animals, p>0.05, and were probably lower than the initial values. Thus, a decrease in tartrate-resistant acid phosphatase in the blood of animals with simulated osteoporosis treated with antiresorptive therapy can be interpreted as a decrease in bone tissue resorption. Conclusion. Materials and methods. Analyses, generalization and systematization of information from scientific and methodical literature were used in the study. Results and discussion. Tutoring technology is one of the credit-module system of education components in educational institutions of the III-IV levels of accreditation. Individualization of education contributed to the development of tutoring technologies, which are aimed at the development of independent work in the educational process of a higher medical school, and the tutor is responsible not only for the level of knowledge, but also for the formation of the personality of the future doctor. The introduction of tutoring into the educational process is a desire to identify the gifted young generation, to help them organize themselves in the educational space of the university and to get to know the applicants with their dreams and problems. The tutor gradually acquires a different role from previous work experience, which requires mastering self-organization techniques, the ability to take the initiative in a situation of uncertainty, and the cultivation of paradoxical thinking. The tutor becomes a role model, showing students certain orientations and techniques for self-improvement. Implementation of tutoring in a higher medical school takes place on the basis of mutual respect and trust, which creates a real opportunity for continuous professional growth of the teacher's competence and increasing the knowledge of students, their self-esteem, forms of flexibility and non-standard thinking, creates a desire to replenish one's own knowledge. Tutoring is one of the promising directions of the development of the individualization of training in the professional training of future medical students, which contributes to: creation of a whole set of educational and teaching-methodical manuals for independent work; development of integrated interdepartmental tasks; development of collegial relations of teachers and students; development of tasks that involve non-standard solutions. Conclusion. Tutoring is one of the forms of organizing the educational process, which involves mostly individual consultations, the use of project methods, and the introduction of information technologies.
