However, there are fewer cases of KD in infants younger than 6 months, making diagnosis more challenging. The objective is to characterize the clinical presentation and evolution of KD in infants < 6 months of age as compared to those ≥6 months in western Algeria. Methods: We retrospectively reviewed the medical records of 62 patients diagnosed with KD in western Algeria followed between January 2018 and January 2023. The data were categorized into 2 groups by age: Group A (<6 months, n=12) and Group B (≥6 months, n=50). We compared differences in laboratory data, clinical presentation, treatment response, and coronary artery outcomes between the two cohorts. Results: The majority (78%) of infants and children ≥6 months of age were initially diagnosed with KD, as compared to only 33,3% of infants <6 months. Clinical features of KD were more commonly observed in the older cohort: oral changes (90 vs. 75%, P = 0.0023), extremity changes (76 vs. 50 %, P = 0.029), and cervical lymphadenopathy (66 vs. 33.3%, P = 0.0004). Whether treated in the first 10 days of illness or after the 10th day, infants <6 months were at greater risk of developing a coronary artery aneurysm compared to KD patients ≥6 months treated at the same point in the course of illness [≤10 days (55 vs. 8,88 %, P = 0.0001) ; >10 days (66,6 vs. 6,6%, P = 0.046)]. Conclusion: Our data show that despite treatment in the first 10 days of illness, infants <6 months of age have a higher risk of developing a coronary artery aneurysm. Delay in the diagnosis leads to larger coronary artery aneurysms disproportionately in these infants. It can lead to debilitating motor and cognitive sequelae. Disease modifying therapies have been used in children as well as adults, with similarities observed in terms of tolerance, and side-effect profiles. At present the consensus in management of Pediatric MS is based mainly upon adult-based treatment trials. Multiple factors are taken into consideration, including disease activity, coexisting morbidities, patient-based preference, and socioeconomic factors. Rituximab, has shown in numerous case series and reports, to be efficient and well tolerated. We report a female child with a relapsing, progressively debilitating CNS demyelinating disease with poor compliance to Disease-Modifying therapies, and breakthrough relapses during Interferon Beta-1b and Fingolimod trials. Their diagnosis yet remains challenging and many underdiagnosed. During the infectious episodes the patients affected with these disorders present with different levels of severity especially affecting the respiratory system. Here, we report a novel mutation in a Lebanese patient presenting with an early onset mucle weakness and motor regression. By Whole Exome Sequencing, a novel likely pathogenic variant in the thymidine kinase 2 ( TK2 ) gene was found confirming the diagnosis of mitochondrial DNA depletion syndrome type 2 (Myopathic type). At age of 4 years and a half, the patient was affected by COVID - 19. The Melody valve has the greatest experience around the world with more than 10,000 implants, mostly in the pulmonary position, recommended for patients over 5 years of age and 30kg. There are reports of hybrid implants through a small subxiphoid incision for those patients who do not meet the requirements for a traditional implant. Material and method: Retrospective observational analysis with demographic and procedural evaluation, and medium - term follow - up of two cases of IVP through the periventricular route, in a tertiary care University Hospital. Results: Both patients had multiple sternotomies and had presented severe neurological complications secondary to corrective surgery, which is why they were rejected for surgical valve replacement; weighed less than 30 kg; They met criteria for pulmonary valve replacement and were proposed for periventricular IVP. In a hybrid catheterization laboratory with support from pediatric cardiovascular surgery, a minimal sternotomy (lower third of the sternum) was performed; Extra support guidewire was stabilized in the left pulmonary branch through a hemostatic purse string and Landing Zone was performed with a CP stent. Subsequently, the Melody valve was implanted on an 18 and 22 mm balloon (case 1 and case 2, respectively). Control angiographies were performed without observing pulmonary insufficiency or paravalvular leak. Both were discharged from the hospital in excellent hemodynamic condition at 72 hours, maintaining periodic check - ups. It can present completely asymptomatically in early childhood, only to end with cor pulmonale in later life. We report two cases - the first, a six - year - old boy who had a strong familial history of lung diseases and presented with a history of chronic cough. No further symptoms were elicited, although two elder familial members had passed away of cor pulmonale in their mid - 30s. The other is an eight - year - old boy referred to as a case of “ non - responsive pulmonary tuberculosis, diagnosed sans microbiological evidence. Respiratory examination, and growth were normal in both children. Chest X - ray had a typical ‘ Stone Lung ’ appearance. The clinical findings were in contrast to the radiological picture and hence a CT scan (Chest) was done. On further suspicion, lung biopsy and genetic analysis established the diagnosis of Pulmonary Alveolar Micro-lithiasis The patients were given supportive treatment and started on oral Etidronate. She was observed with dysmorphic features in the form of upward slanting eyes, short neck, sandal gap de-formity sign [Figure 1], but there was no signs of hydrops fetalis. Rest of the systemic examination was normal. The blood gas analysis revealed severe mixed acidosis with pH 6.8, PaO2 90 mmHg, and PaCO2 113 mmHg, HCO3 and BE were un-recordable. The baby was connected to conventional mechanical ventilation; IV fluids and antibiotics were charted but there was progressive worsening with increasing Fio2 requirements up to 100%. An urgent bedside chest X - ray was done revealed left side pleural effusion with mediastinal shift of the airway and heart to the right side [Figure 2]. Echo-cardiography was done and showed large sized patent ductus arteriosus with severe persistent pulmonary hypertension. Aim: This study aimed to describe the epidemiology, pattern, duration and the results of treatment of poisoned patients who were seen in Dubai and Latifa hospitals in Dubai, United Arab Emirates UAE, between the months of September 2017 and September 2019. Additionally, the study examined possible risk factors for childhood poisoning, response to and management of poisoning emergencies. Methodology: This retrospective descriptive study is a hospital - based study which analysed clinical profiles of children of 13 years and below taken at two health care centres within the 2 years of study for treatment of poisoning complications. Result: The study found that out of 603 poisoning cases recorded, 579 patients met the inclusion; the majority of the patients were (n=355; 61.3 %) males and (n=224; 38.7%) females. Data written on the data collection form were transferred into excel and later into SPSS. The data were analysed using frequencies and percentages, and a chi - square test was used for categorical variables. The most common toxic substances ingested by children were cosmetic and personal care products (n=130; 22%). The recorded admissions in the course of research period were 104 which accounted for 18 % of the total poisoning cases. Conclusion: This research concluded that the most common toxin ingested was personal care products and detergents. Poison ingestion was more predominant in males and children below 5 years of age.
