Dengue infections can result in a broad spectrum of disease severity ranging from an influenza-like illness (DF) to the life-threatening dengue hemorrhagic fever (DHF)/dengue shock syndrome (DSS). Although adults do experience shock, vascular leakage is generally more severe in young children, and in endemic areas, DSS is seen primarily in the pediatric population. The detailed clinical and laboratory profile is crucial for the diagnosis and the successful management of the patients.  This study aims to observe the clinical features and laboratory results for the better understanding of patient management to reduce child death in case of dengue fever. A prospective cross-sectional study was conducted at the dengue ward of Khulna Medical College Hospital; 167 suspected dengue patients were enrolled in our research during the period from 7th August to 8th November 2019. Patients attending the emergency department and pediatric outpatient department with complaints of fever and features of dengue with positive NS1 antigen test and/or positive dengue specific antibody IgM or IgG or both were included in this study. Diagnosis of dengue was made on the basis of compatible clinical features and NS1 antigen positivity and/or detection of IgM and IgG antibodies. Patients were classified according to WHO 2012 protocol, and data was analyzed.  One hundred sixty patients (95.8%) were diagnosed as having dengue illness, and the rest 7(4.2%) as fever other than dengue. Among the dengue illness, dengue fever (DF) 87(52.1%), DF with warning signs 51(30.5%), DHF 19(11.4%), and DSS 3(1.8%).The most common sign-symptoms of dengue was fever(100.0%),vomiting(52.1%),abdominal pain(38.3%), headache(23.4%),skin rash(13.25%), melena (7.8%), abdominal tenderness(28.7%),palpable liver (24.6%),and palpable spleen(3.0%). NS1 antigen was found positive in 88.6% cases, negative 3.0%, and NS1 antigen test not done 8.4% cases due to time constrain. Anti-dengue antibody IgM positive in 10(6.0%),IgG positive 7(4.2%),both IgM & IgG positive 7(4.2%),both IgM & IgG negative 57(34.1%) and antibody test not done in 86(51.5%) cases due to time constrain. However, some authorities are of the opinion that excision of the hernial sac without ligation is not associated with recurrence of the hernia. Some have even shown adverse events related to hernial sac ligation like increased postoperative pain and discomfort. There is no consensus on how the hernial sac should be managed during laparoscopic herniotomy. Objectives: Present study aimed to determine the feasibility of laparoscopic sac excision without ligation in pediatric age group, by a randomized comparison. Materials and methods: This interventional study was performed in the department of Paediatric Surgery, Rajshahi Medical College Hospital, Rajshahi, Bangladesh, over a period of one year. A total of 189 patients were included in the study as per inclusion and exclusion criteria and randomized in the three groups. Comparisons were made among three procedures of laparoscopic herniotomy, namely; Percutaneous extraperitoneal closure, Intraperitoneal pursestring closure and laparoscopic sac excision without ligation. The main outcome measures were recurrence rate, operative time, hospital stay, postoperative hematoma and hydrocele formation. Results: Recurrence rate, operative time and hospital stay were not amplified in laparoscopic sac excision procedure than others. Postoperative hydrocele formation was significantly less. Conclusion: Laparoscopic sac excision in indirect pediatric inguinal hernia is safe. Data were collected by face to face interview, clinical examination. The study revealed that the colorectal carcinoma was highest in the 5th and 6th decade and rectal area (46.3%) and male predominance was observed with male to female ratio being 3:2. Abdominal pain (61.1%), weight loss (61.1%), per rectal bleeding (57.4%), altered bowel habit (24.1%) and melena (24.1%) are the main presenting symptoms. Prompt diagnosis and early initiation of treatment is necessary to prevent long term complications. Knowledge about bacteriological profile of pediatric urinary tract infections and their antimicrobial sensitivity pattern is helpful for initiation of empirical antibiotic treatment. The aim of this study was to identify the causative bacteria and antimicrobial sensitivity pattern of community acquired UTI in children attending outpatient department of a tertiary level hospital of Bangladesh. This was a cross-sectional study conducted in pediatric outpatient department of Rajshahi Medical College Hospital, Bangladesh over 6 month period from 1st January 2017 to 30th June 2017. Children aged < 12 year who presented with clinical features suggestive of urinary tract infection were subjected for urinary routine and microscopic examination and culture. Colony counts of the culture positive cases were done and antibiotic sensitivity and resistance patterns were studied. Samples with colony count of ≥ 105 CFU/ml were considered significant. Among the 134 clinically suspected cases significant bacteriuria was found in 81 (60.45%) children. Escherichia coli was the most common isolate (74.1%) followed by Proteus (9.8%), Klebsiella spp. (8.6%), Pseudomonas (6.2%) and Staphylococcus saprophyticus (1.2%). E. coli was found to be highly sensitive to imipenem, amikacin, nitrofurantoin and ciprofloxacin. Klebsiella showed good sensitivity against ciprofloxacin and imipenem whereas proteus, Pseudomonas and Staphylococcus saprophyticus showed sensitivity against imipenem, amikacin and ciprofloxacin. Increased resistance was noted against the commonly used empirical antibiotics such as cephradin, cefuroxime, cefixime, ceftriaxone and co-trimoxazole. Repeated Hyperlipidemia and frequent long term steroid treatment in relapsed cases add more burdens on already fragile health of these children. Materials & Methods: This descriptive, observational study was carried out in the Department of Pediatrics, Rajshahi Medical College Hospital over a period of 24 months from July 2015 to June 2017. If a child’s age at presentation was between 2 to 8 years of age group of either sex having typical features of nephrotic syndrome(first attack), such asgeneralized swelling which started from the face, having abnormal urinary finding ( proteinuria 3+ or more and UTP >1gm/m/24hrs hypoalbuminemia, hypercholesterolemia but responded to steroid was included in the study. When the child was in remission, serum Lipid profile was done following 10 hours of fasting once again. Then the child was followed up fortnightly for a period of six months from the initial attack for generalized edema and bedside urinary albumin 3+ or more. Results were tested with independent ‘t’ test and χ test to find out the association between high lipid profile during remission with the relapse . A two-tailed P value of 0.05 was considered statistically significant. Results: All 50 children were divided into two groups on the basis of lipid profile during remission. Group-1 consists of 20 children who had normal lipid profile during remission and group-2 consists of 30 children who had abnormal lipid profile during remission. Both group-1 & 2 had higher mean levels of serum cholesterol, LDL, and TG levels during initial diagnosis. During remission, group-2 patients showed higher mean serum cholesterol (364.60±64.00 mg/dl vs. 189.25±9.02 mg/dl), serum LDL (241.53±58.42 mg/dl vs. 146.65±3.60 mg/dl), and serum TG (225.10±43.79 mg/dl vs. 138.90±8.15 mg/dl). All these differences in lipid profile between these group were statistically highly significant (p=<0.001).Only 2(10%) cases underwent relapse from group-1 out of 20 cases, whereas 16(53.33%) cases underwent relapse out of 30 cases from group-2 within six (6) months follow-up. The difference between group-1 & group-2 was statistically significant (p=0.002).Among the relapsers, mean cholesterol (394.18±43.03 mg/dl vs. 329.50±67.81 mg/dl; p= 0.004) was significantly higher than that of non-relapsers of group-2 patients. Conclusion: Elevated lipid level, especially serum cholesterol during remission phase may be associated with subsequent relapse in idiopathic childhood nephrotic syndrome. In some high risk pregnancy placental maturity is accelerated as in pregancy –induced hypertension (PIH), intrauterine growth retardation (IUGR), whether in other high risk cases like diabetes and Rh-isoimmunization there is delayed placental maturation. \u0000Objectives: This was a cross sectional type of descriptive study. The study was carried out to assess placental grading by USG in high risk and normal pregnancy for predicting neonatal outcome. \u0000Material and method: The study was carried out in the Department of Obstetrics & Gynaecology in collaboration with the Department of Radiology & Imaging of Rajshahi Medical College Hospital from January 2012 to December 2013. : A total 200 pregnant mother, attending the inpatient department constituted study population who delivered their babies at Rajshahi Medical College Hospital were selected in antenatal period for USG examination to detect placental grade. \u0000Result: Ultrasonography showed, 35% had Grade-II placental maturity, 33.5%- Grade-III, 25% Grade - I and 6.5% had Grade-0 maturity. Majority of the mother (75%) were normal and had healthy baby. The relationship of placental maturity with gestational age of mother in normal pregnancy and in high risk pregnancy was found to be statistically significant. Placental maturity and fetal outcome in normal pregnancy was found significantly associated (p<0.001) but in high risk pregnancy it was not associated significantly (p>0.5). Since the introduction of the first flexible fiberoptic bronchoscope (FOB) has been a valuable tool in the diagnosis of pulmonary diseases. \u0000Material & methods: This study was conducted in Rajshahi Medical College Hospital (RMCH), Rajshahi. It was undertaken to study the role of fiberoptic bronchoscopy (FOB) in diagnosis of pulmonary disease. The study period was between 1st June 2017 to 30th June 2019. A total number of 300 patients (48 females and 252 males) who underwent elective fiberoptic bronchoscopy were included in this study. Data was collected from hospital record regarding age, clinical presentation, diagnostic procedure and outcome. \u0000Results: A total number of 300 patients had fiberoptic bronchoscopy during the study period. 252(84.00%) patients were male and 48(16.00%) were female. As per age distribution 8(2.67%) were of age group 20-32 years and majority 111(37.00%) of patients were in age group 60-72 years. Min age group was 20 and max 85 and the mean±SD age was (58.70±13.11). Occupation of the study participants most of them are farmer 134(44.67%). Bronchoscopy findings: most of the patients had malignant lesion 228(76.00) and a few had foreign body 6(2.00%), mean±SD (2.03±0.68). Majority of the patients had problems in their upper lobe 167(55.67%) and some of the patients had problems in lower lobe 22(7.33%), mean±SD (0.97±0.15). Regarding location, left principal bronchus were involved in 115(38.33%), upper lobe 95(31.67%), lower lobe 36(12.00%) and lingular lobe 36(12.00%), mean±SD (2.01±1.02). Trachea was found normal 261(87.00%) and growth 29(13.00%), mean±SD (1.13±0.33). Vocal cord found normal 246(82.00%) and palsy was found in 54(18.00%), mean±SD (1.18±0.38). carina findings were normal 54(18.00%), blunt 230(76.67%) and sharp 13(4.33%), mean±SD (1.26±0.50). \u0000Conclusion: FOB can be successfully employed for the diagnosis of lung diseases, including malignancies and granulomatous lesions. The hand is prone to domestic and industrial trauma with fingertips being the most frequently injured portion of the hand. Fingertip injuries are commonly seen by emergency physicians \u0000Objectives: To see the functional outcome after V-Y flap in fingertip injuries. \u0000Methods: Prospective, observational study carried out for 2 years at National Institute of Traumatology and Orthopaedic Rehabilitation (NITOR). Fifteen patients of fingertip injuries were selected for V-Y flap. \u0000Results: V-Y plasty was performed in 15 patients who suffered from a transverse amputation of the finger tip. Out of 15 patients, functional outcome was satisfactory (good) in 13(86.7%) cases and unsatisfactory (fair plus poor) in 2 cases (13.3%). 3 patients had postoperative complications. Early detection of obesity and identification of risk factors are the key to successful prevention of obesity. Objectives: This study was conducted to determine the proportion of obesity and presence of risk factors in obese children in primary schools of Rajshahi city. Methodology: This cross sectional study included total 225 children chosen from different primary schools of Rajshahi city during the school year 2016-2017. Data collection tool used for the study was an interview schedule and this predesigned questionnaire. The main outcome measures were obesity and risk factors prevalent in the obese school children. Data were analyzed by SPSS version 20 and ‘Chi square’ test was applied for the test of significance. Result: Among the total 225 respondent, 121 (53.8%) were male, while 104 (46.2%) were females, with a mean age of 7.9 ±1.1 years. The mean and standard deviation of anthropometric measurement were weight 22.7±5.3 kg, height 120±8.1 cm and BMI 15.5±2.1 kg/m. Out of 225 participants, 2.7% (total 6) were overweight, 5.3% (total 12 students) were obese, 83.6% (total 188 students) were normal weight and 8.4% (total 19 students) were underweight. The rate of obesity was highest at the age of 8-9 years (10.7%) and overweight were highest at the age of 910 years (5.5%). Playing, biking regularly was strongly associated with fewer incidence of overweight and obesity (p value -.001). Faulty dietary habit, having more fast food, chocolate, sugary juice lead to a higher BMI and more fresh fruit and vegetables had lower BMI (p value .001). Conclusion: This study shows that childhood obesity is on rise in our country. Addressing the problem at its earliest could be achieved through identifying high risk groups and planning early intervention.
