EFFICACY AND SAFETY OF PRIMARY CLOSURE OF COMMON BILE DUCT FOLLOWING LAPAROSCOPIC CHOLEDOCHOLITHOTOMY 腹腔镜胆总管取石术后一期闭合胆总管的疗效和安全性

Several surgeons perform open CBD exploration with primary duct closure. However, few surgeons perform duct closure laparoscopically due to technical difficulties and concerns about missed stones and postoperative complications. Thus, we evaluated the efficacy and safety of the primary closure of the common bile duct following laparoscopic choledocholithotomy. In this prospective observational study, 62 patients with CBD stones were confirmed pre-operatively by abdominal ultrasonography and/or magnetic resonance cholangiopancreatography (MRCP). CBD radiological findings and demographics were noted. Details on the intraoperative technique, overall operating time, and post-operative complications were observed. Patients were followed for one month, and post-operative data were collected and analysed. Most of the enrolled patients were female (69.35%), aged 41-50 (37.10%), and had abdominal pain (82.25%). 64.52% had multiple stones in the CBD of the calibre of 8–10 millimetres (40.32%). 94.52±21.58 mins was the mean operative time. CBD clearance was noted in 96.77% of cases. In most cases, intraoperative bleeding was 51–100 ml. Laparoscopic primary closure was performed in 96.77% of patients. On days 3 and 7, 9.68% and 6.45% of patients had bile leakage, respectively. The removal of abdominal drains varied significantly between cases. During the follow-up, two patients with bile leaks and jaundice after surgery were found to have retained stones and underwent post-operative ERCP without additional complications. The major prognostic factors are degree of differentiation and depth of invasion in urothelial tumors. Ki-67 is a non- histone nuclear protein marker of cell proliferation which is encoded by MKi-67 gene in humans. This study evaluates and correlate immunohistochemical Ki-67 expression in Non-invasive & Invasive urothelial neoplasm. This retrospective study is done in the Department of Pathology of L.N. Medical College and J.K. Hospital, Bhopal. Urothelial Neoplasm cases diagnosed from January 2019 to January 2022 were included, & relevant clinico-pathological data and their Ki-67 Labelling Index was evaluated & correlated. Total 60 cases were studied, 30 had invasive carcinoma whereas, 30 had non-invasive neoplasm. Ki-67 expression (>13%) was seen maximum in invasive cancers i.e 40% cases, whereas (<13%) was seen maximum in non-invasive neoplasms i.e 38.3%. A statistically significant expression of Mean Ki-67 Labelling Index was observed that increased from papilloma to PUNLMP, Non- Invasive urothelial neoplasm Low grade & High grade in non-invasive urothelial neoplasm (p<0.001), and from lamina propria invasive to muscle invasive urothelial cancers (p=0.013). A Higher tumor proliferation of greater than 13% was significantly related to greater tumor size (p=0.04). Ki-67 labelling index being a measure of tumor proliferation is related to tumor histological grade. Large urothelial tumor size was associated to high Ki-67 LI and was not strongly associated with age and gender. There has been growing interest in the potential anti-cancer properties of Delta-9 tetrahydrocannabinol. Gemcitabine, a nucleoside analog, has shown effectiveness against diverse cancer types. This study aimed to assess the combined efficacy of Delta-9 tetrahydrocannabinol and Gemcitabine in treating cancer. The squamous lung cancer cell line required for our study was provided by the stem cell unit. The doses were determined as 5µm/L and 10µm/L for Delta-9 tetrahydrocannabinol, 20 and 40 µm/Lfor Gemcitabine by performing a literature review. In our study, CellViabilityAnalysis by MTT, Xcelligence Real-Time Cell Analysis, Annexin V Apoptosis Flow Cytometry Analysis, Total Oxidant, Antioxidant Status, and Caspase-3 Detection Analysis was performed. Upon evaluating the rates of apoptotic cell death, it was observed that the THC 5 and THC 10 treatment groups exhibited a 30% and 60% increase, respectively, compared to the alcohol group. A significant difference in cytotoxic effect, as determined by MTT, was found between the control group and the Gemsitabin 20, Delta-9 Tetrahydrocannabinol 5µm/L, and Gemsitabin 20+Delta-9 Tetrahydrocannabinol 5µm/L groups (p<0.001). There was a statistically significant difference between the groups regarding TOS (p<0.001). All experimental groups exhibited a higher level of caspase 3 activations, in comparison to the control group. We observed a significant cytotoxic effect of Gemcitabine on squamous cell lung cancer cells. Delta-9 tetrahydrocannabinol, when used alone, exhibited a relatively low cytotoxic effect. The scientific community has many obstacles in tackling this problem and creating a streamlined therapy and drug administration plan. Chemotherapy is necessary for cancer treatment, however many individuals acquire drug resistance due to recurrent medication delivery. Chemotherapy resistance hinders cancer treatment. Exosomes are vesicles their size range from 40 to 160 nm (the average being 100 nm). Exosomes can include DNA, RNA, fatty acid, metabolites as well cytosol, and cell surface proteins. Since exosome formation's physiological purpose is unknown, more research is needed. Exosomes regulate intercellular communication and help cancer cells develop chemoresistance. Exosomes can reveal physiological and pathological information about their parent cells. Liquid biopsies assess cancer chemosensitivity using exosomes. Because exosomes are stable and sensitive. This review covers exosome-mediated cancer chemoresistance. To identify and isolate P. aeruginosa, each of them underwent direct microscopy and culture. In this study 63 P. aeruginosa were isolated and identified out of 1200 clinical samples. Pus and urine were where the majority of P. aeruginosa were isolated from specimens. There found sensitivity to Piperacilin- tazobactam (10µg) 35% and Gentamycin (10µg) 38% respectively by kirbey-Bauer disc diffusion method. Vetiver has a diverse historical background in traditional medicinal practices; however, only a limited number of research articles have documented its efficacy in disease treatment. The objective of this review was to collate current data about the phytochemistry, pharmacology, and customary applications of Chrysopogon zizanioides (C. zizanioides). The primary objective of the manuscript is to integrate pertinent, genuine, and current information. Relevant articles were also extracted from online databases, including PubMed, Scopus, and Science Direct. The findings of the compilation process indicate that the efficacy of vetiver's traditional uses has been confirmed through the testing of different extracts of this plant part against disease-based pharmacological models. Nevertheless, further comprehensive research is required to substantiate the extraction of bioactive compounds and elucidate their mode of operation. HGS is influenced by basic anthropometric determinants such as age, stature, and gender. The information about the differences in these anthropometric parameters in athletes as compared to non-athletes and the consequent influence on HGS is scarce. Investigations of the association between involvement in hand-related and non-hand-related sporting activities among different age groups of both sexes with HGS, particularly in a developing nation like Nigeria are scarce. This study aimed to investigate how anthropometric parameters of age, height, weight, and body mass index (BMI), affect handgrip strength and their association with sport-related parameters. A crosssectional study was conducted on 370 healthy Nigerians aged 17-40 years, from sports centres in Kwara state. Subjects were grouped into hand-athletes, non-hand-athletes, and a control group of non-athletes. A questionnaire was administered to get their biodata, while measurements of HGS(lbs); using a dynamometer, height(m), weight(kg), and BMI(kg/m2) were taken. HGS of hand-related athletes was significantly greater than the non-hand athletes and non-athletes. No significant difference was observed between the non-hand athletes and non-athletes. HGS of the non-sportsmen was significantly correlated with age, height, weight, and BMI, however, there were no significant correlations for non-sportswomen. HGS of the non-hand-sportsmen was significantly correlated with age, weight, and BMI, but not with height, while only BMI was significantly correlated for non-hand-sportswomen. HGS of both hand-sportsmen and hand sportswomen were significantly correlated with weight and BMI, however, age and height showed no significant correlation. The data obtained were analyzed using the SPSS 23.0 software by creating the database. The stat൴st൴cal s൴gn൴f൴cance level was set at 0.05. The mean scores the participants obtained from the Dyadic Adjustment Scale and Sexual Quality of Life Questionnaire-Female were 106.27±22.21 and 77.46±21.85 respectively. Of the participating women, 36.7% used an intrauterine device for birth control, 27.8% used condoms, 13.8% used hormonal drugs. No statistically significant differences were determined between the mean scores obtained from the Dyadic Adjustment Scale and between the mean scores obtained from the Sexual Quality of Life Questionnaire-Female in terms of the family planning methods the participants used (p>0.05). However, differences between the mean scores the participants obtained from the affectional expression subscale of Dyadic Adjustment Scale were found to be statistically significant in terms of the family planning methods they used (p<0.05). While the marital adjustment was high among the participating women their quality of life level was moderate. It was determined that the sexual quality of life increased as the marital adjustment increased. It was also determined that family planning methods the participants preferred affected not their sexual quality of life and marital adjustment but the affectional expression subscale.
