This research seeks to determine perioperative mortality rate in the surgical wards of Aliabad Teaching Hospital, Kabul, Afghanistan. The cross-sectional study was conducted after obtaining the ethical approval from Scientific Research Center review committee in Aliabad Teaching Hospital from March 2020 to March 2021. Patient data were collected from medical archive. Anesthesia and surgical log books were also used for collection of the data. Data were initially entered into an excel data sheet, and then exported to SPSS Statistics version 24 for further analysis. From the overall 1832 participants, 1050 were males and 782 females. 60.97 % of the patients had received general anaesthesia. Both elective and emergency procedures were included in the study. 37.9 % of the patients had systemic diseases. The perioperative mortality rate was found to be 3 % among the study participants. Trauma was the first significant reason of mortalities in the study. Majority of the mortalities have occurred in patients undergoing general anesthesia, being a young adults and emergency procedures. The differential diagnosis of neuromyelitis optica and multiple sclerosis is based on the identification of a typical clinical and tomographic picture and the presence of a high level of antibodies to aquaporin 4 in the blood serum. We presented a familial clinical case of Devic`s disease in a representative of the indigenous population of Central Asia and the associated difficulties in differential diagnosis. Aim: This study aims to determine the prevalence of extramedullary manifestations at the initial presentation of acute lymphoblastic leukemia in children. Method: This is a retrospective, cross-sectional study assessing medical records of 73 patient newly diagnosed (ALL) at Almaty Scientific center of pediatrics and surgery during-2020. The children had a mean age of 6.9 ±4.27 years. The female and male patients made 46 (63 %) and 27(37 %) respectively. children with acute lymphoblastic disease were classified into two subtypes (B-cell ALL and T-cell ALL) according to the WHO classification. Data earned from the study were enrolled within a directory also statistical analysis was done by applying the software (SPSS) version 26.0. Categorical variables resembled using the chi-square test or Fisher’s exact test, as appropriate. Result: In this study, the most common extramedullary initial signs and symptoms of ALL were related to hepatic system 49 (67.1 %) and gastrointestinal symptom 43 (58.9%).The additional extramedullary presentations of ALL in order of frequency include; lymphadenopathy 20 (27.4%) , musculoskeletal system 19 (26%) urinary system 9 (12.3 %), Respiratory 9 (12.3 %) , Neurological system 5 (6.8%) orbital presentation 3 (4.1%), neurologic signs 8 (9%%), pericardial involvement 1 (1.4%), and the other miscellaneous presentations 3 (4.1%).Conclusion: This study showed that a significant number of ALL patients present with variety of extramedullary symptoms that pediatrician must become familiar with those to detect some atypical cases of leukemia early on. About 90% of cholecystectomies are performed laparoscopically due to faster recovery, fewer complications, cost and cosmetic effect. In this study, we evaluated the features of laparoscopic cholecystectomy in general, as well as in obese patients, aged patients, and pregnant women. Carrying out LCE in the elderly, pregnant women and obese patients today does not cause big problems. The main thing is to take into account the characteristics of each patient. In the case of LCE in the elderly, careful preoperative preparation of the vascular, hemodynamic, and respiratory status should be carried out to prevent adverse events. And the optimal period for the safest laparoscopic surgery in pregnant women is the second trimester. It is also necessary to remember that proper preparation should be carried out in consultation with obstetricians and anesthesiologists. At the same time LCE can be performed safely and effectively in patients with even a high degree of obesity showing equally good results compared to patients with normal BMI. Laparoscopic cholecystectomy is safe in patients with Child-Pugh A cirrhosis and should be used with caution in patients with Child-Pugh B cirrhosis. The manifestation of the disease is characterized by many clinical, laboratory and instrumental symptoms found in acute and chronic cholecystitis. Among them, it is worth noting pain in the right subcostal region, the jaundice, increased body temperature and symptoms revealed during ultrasound. However, in some patients, the disease is asymptomatic and its first manifestation may be obstructive jaundice. Because of the presence of gallstones and jaundice, they are often confused with other obstructive conditions. Preoperative diagnosis is complex and often overlooked. The difficulties in diagnosing Mirizzi syndrome, the high risk of damage to the biliary tract, as well as the wide spectrum of surgical treatment determine the relevance of this study. A literature review has been made on the topic with the clinical situation analyzed. A 62-year-old patient was admitted with a diagnosis of biliary tract obstruction. The patient was diagnosed with gallstone disease.
