A 16-year-old male of mixed ethnicity presented with gradual loss of vision and pain on eye movement in his left eye 12 days after his second Pfizer vaccine. His visual acuity in the left eye was count fingers at half a meter. There was optic nerve head swelling, macular exudates in a star-shaped pattern with peripapillary and foveal subretinal fluid consistent with left neuroretinitis. His serological and radiologic investigations were negative for any alternative aetiology. He was commenced on doxycycline, rifampicin and oral prednisone on a tapering dose. 4 months after presentation his left eye vision improved to 6/24-2; limited by loss to the outer retinal layers over the fovea. Woźniak, Weronika Kawecka","doi":"10.5603/oj.2023.0010","DOIUrl":"https://doi.org/10.5603/oj.2023.0010","url":null,"abstract":"Agents blocking vascular endothelial growth factor (anti-VEGF) — aflibercept, bevacizumab, and ranibizumab are commonly used drugs in the treatment of retinal vascular diseases, including age-related macular degeneration, diabetic retinopathy, retinal vascular occlusions and retinopathy of prematurity. To date, intravitreal injection is the only successful administration method of anti-VEGF agents. Each administration can potentially lead to rhegmatogenous retinal detachment, intraocular pressure elevation, ocular hemorrhage, and endophthalmitis. Endophthalmitis is a rare complication, occurring in 0.012–0.1% of cases of anti-VEGF injections. The most frequent isolated pathogens are Staphylococcus spp. And Streptococcus viridans — commensals of the human upper respiratory and oral flora. The main symptoms of endophthalmitis are pain and decreased visual acuity. Patients become symptomatic on average three days after the injection. Prevention of endophthalmitis includes sterilization of ocular surface with povidone-iodine, use of sterile gloves, use of eye speculum, and „no-talking” policy. Topical antibiotics are not routinely used as they can even increase the risk of post-injection endophthalmitis. Similarly, renal cell carcinoma (RCC) is another rare malignancy that counts for 2–3 percent of all adult malignancies. Second primary cancers are not uncommon, even though they occur less frequently than primary cancers. CASE PRESENTATION: We present a rare choroidal melanoma case in a 55-year-old adult who presented with a one -month history of the painless gradual decrease of vision. His investigations revealed a clinical diagnosis of large choroid melanoma, and upon metastatic workup, he was incidentally found to have synchronous RCC. Both malignancies were treated surgically (and the pathology confirmed the clinical diagnosis of choroidal melanoma and RCC).
