{"title":"COMPARATIVE STUDY OF POLYMERIZATION DEPTH OF THREE PHOTOCOMPOSITE DENTAL FILLING MATERIALS FOR BULK FILL RESTORATION","authors":"I. Noenko, O. Three flowable bulk fill photocomposite dental restorative materials – \"SDR® Posterior Bulk Fill Flowable Base,\" \"Filtek™ Bulk Fill Flowable\" and domestic \"Jen-Radiance Molar-FW\" – were chosen for the study. For each material, 10 black opaque thin-walled polyvinyl chloride molds with a length of 10.00 mm and a diameter of 3.00 mm were prepared. The molds were air-dried from a compressor and fixed in fluoroplastic retainers perpendicular to the metal opaque surface below and subsequently filled with the investigated restorative material along the entire length, in a contact manner, along the wall. After filling, the material was polymerized with an LED photopolymerizer directly in contact with the surface of the light guide for 40 seconds. Subsequently, the material was removed from the forms and the remains of the unpolymerized mass were removed from the surface of the polymerized cylinder of the composite with a metal dental sickle-shaped carrier (scarification test). The length of the polymerized cylinder was measured using an electronic micrometer.\u0000Results. Therefore, the study of the depth of polymerization of flowable bulk fill photocomposites \"SDR® Posterior Bulk Fill Flowable Base\", \"Filtek™ Bulk Fill Flowable\" and domestic \"Jen-Radiance Molar-FW\" showed that signs of polymerization of all three materials were observed in a depth of more than the \"declared\" four millimeters which may be explained by the improvement of the recipe for the preparation of such materials and the optimization of the transparency of the organic component and inorganic filler, as well as the regular distribution of photoactivators in the mass. \"SDR\" polymerized to a depth of 7.38 ± 0.17 (M = 7.30) mm, the minimum value was 7.16 mm and the maximum value was 7.71 mm. \"Filtek Bulk Fill Flowable\" polymerized to a lesser depth – on 6.34 ± 0.14 (М = 6.38) mm, the minimum value was 6.05 mm and the maximum value was 6.55 mm. The domestic analogue \"Jen-Radiance Molar‑FW\" showed even higher values of polymerization depth – 8.03 ± 0.19 (М = 7.98) mm, the minimum value was equal to 7.75 mm and the maximum value was 8.41 mm. Statistical tests showed probable similarity between \"SDR\" and \"Jen-Radiance Molar-FW\" in the depth of polymerization.\u0000Conclusions. All three studied flowable bulk fill photocomposite materials were polymerized to a depth greater than 4.00 mm. Thus, new medication compositions are one of the ways to solve this problem. This study aimed to investigate the antimicrobial efficacy of the citrate buffer combined with the antibiotic «Amoxiclav» on standard reference strains of microorganisms.\u0000We used standard cultures of C. albicans ATCC10231, E. coli ATCC25922, S. aureus ATCC25923, E. faecalis ATCC29213, M. luteus ATCC4698, S. epidermidis ATCC14990 in the study conducted at the Microbiology, Virology and Immunology Department of the Poltava State Medical University. The sensitivity of standard microorganism strains to the composition was studied with a quantitative method of serial dilutions based on CLSI. ISO/TC 212 «Clinical laboratory testing and in vitro diagnostic test systems. 2021».\u0000Thus, the results of this study clearly showed a synergistic effect of citrate buffer and amoxiclav on all the reference strains of microorganisms. Reference strains of Staphylococcus aureus and epidermal staphylococcus showed the highest sensitivity to citrate buffer, which was 4 times (p < 0.05) higher than that shown by micrococci, enterococci and fungi, and 8 times (p < 0.01) higher than that of Escherichia coli, which appeared least sensitive to the test substance. Amoxiclav also inhibited the growth of S. aureus ATCC 25923 and S. epidermidis ATCC 14990 and had the minimal effect on the reference strain of E. coli ATCC 25922, the minimal inhibitory concentration of which was 31.3 times (p < 0.0001) higher than that of staphylococci. The proposed citrate buffer-amoxiclav combination significantly increased the antimicrobial efficacy of its components against all the reference strains of microorganisms. Thus, the sensitivity of E. coli ATCC 25922 and M. lysodeikticus ATCC 4698 to the proposed combination increased 8-fold (p < 0.05) compared to their sensitivity to citrate buffer or amoxiclav alone. The sensitivity of Staphylococcus aureus and enterococcі to the composition increased to the maximum: up to 32-fold (p < 0.001) to the buffer and 4‑fold to the antibiotic (p < 0.01). The effect of a significant increase in the sensitivity of C. albicans ATCC10231 strain to the citrate buffer after adding amoxiclav was gone up 128-fold (p < 0.0001). {"title":"FEATURES OF SUPERFICIAL VEIN THROMBOSIS IN PATIENTS WITH A LARGE DIAMETER OF GREAT SAPHENOUS VEIN","authors":"V. Mishura, Y. Superficial vein thrombosis is characterized by the formation of thrombi in the superficial veins with subocclusion or occlusion of the venous lumen and its inflammatory reaction, which occurs more often in the lower extremities. Chronic venous disease in 75–88% of cases is the most important clinically identified factor in the development of superficial vein thrombosis. The great saphenous vein is affected in 60–80% of cases. A population-based study found that the chronic venous disease is a risk factor for venous thromboembolism and correlates with an increased risk of mortality in patients. Venous thromboembolism is a major burden of the disease worldwide, with approximately 10 000 000 cases per year.\u0000Objective: to assess the prevalence of superficial vein thrombosis in patients with chronic venous disease and to identify the relationship between the diameter of the great saphenous vein and superficial vein thrombosis.\u0000Materials and methods. Total of 925 chronic venous disease cases were analyzed from January 2019 to December 2021 at the Clinical Department of Surgery, Traumatology, Orthopedics, and Phthisiology of Sumy State University (Sumy Laser Clinic, LLC) for the prevalence of superficial vein thrombosis in patients with chronic venous disease who were undergoing treatment. The patients with superficial vein thrombosis were examined for the diameters of great saphenous vein and venous reflux using ultrasound 10 mm below the sapheno-femoral junction, in the upper and lower thirds of the thigh.\u0000Results: Of 925 chronic venous disease cases, superficial vein thrombosis was observed in 53 cases, which accounted for 5.73 % of the total. Women accounted for 67.9 % (36), men – for 32.1 % (17). The study included patients aged 25 to 69 years (mean age 52.62 ± 10.48 years). In 69.8 % (37) of superficial vein thrombosis cases, the diameter of great saphenous vein was ≥ 10 mm at a level 10 mm below the sapheno-femoral junction; in 49 % (26) of cases – in the upper third of the thigh; in 30.2 % (16) of cases – in the lower third of the thigh. That is, the extension of the sapheno-femoral junction trunk to the lower third of the thigh was preserved in 43.24% of cases.\u0000Conclusion. Fedir Fedorovych (Georg-Friedrich Friedrichovych) Mering was one of the leading specialists in internal medicine in the Russian Empire in the second half of the 19th century. However, his scientific career started not at the Therapeutic Department, but at the Department of State Medical Science. Our analysis of the literature showed that not all stages of F. F. Mering's life were equally well explored. The objective of this article, first of all, was to focus on the coverage of this period in the biography of a prominent Kyivan professor-therapist of the second half of the 19th century. To do this, we will use new historical sources: published and archival ones (such as the curriculum vitae of F. F. Mering).\u0000Results. From September 1/13, 1853, F. F. Mering as an adjunct headed the Department of State Medical Science at the University of St. Vladimir, and from November 4/16, 1854 till December 13/25, 1857, he worked as an extraordinary professor at this department. At the Department of State Medical Science, F. F. Mering lectured on the following subjects: 1) forensic medicine; 2) medical police; 3) hygiene; and 4) the doctrine of epizootic diseases. At the same time, Prof. Mering started lecturing on the history of medicine (since 1854). In addition, from May 31/June 12, 1855, he was in charge of the therapeutic hospital clinic at the Kyiv Military Hospital; in connection with this, on January 29/February 10, 1856, he was appointed junior intern at this hospital. In connection with the relocation of Prof. Mering to the Department of Special Pathology and Therapy in 1857, F. F. Erhardt took the place of an adjunct, and since 1859, the latter was the last extraordinary professor at the department. During these years and until the end of the 1860–61 academic year, Prof. Mering continued lecturing on medical police and hygiene, and F. F. Despite the overall positive epidemiological trend, the burden of tuberculosis in Ukraine remains significant. The most common form of extrapulmonary TB is tuberculous pleurisy. The share of the latter in the structure of all tuberculosis forms is within 10–12% or more. The clinical picture of specific pleurisy is diverse, so its diagnosis is often complicated and ultimately leads to various negative consequences. The authors suggested using a diagnostic system based on ultrasound findings to detect pleural effusion syndrome.\u0000Methods. We examined 329 patients with tuberculous pleurisy, who were divided into two groups. There were 142 subjects in the main group and 187 subjects in the comparison group. The radiodiagnostic examination was based on traditional chest radiography. The patients of the main group were examined according to the pleural diseases diagnostic algorithm implemented by the fellows of the department (utility model patent of Ukraine No. 114430).\u0000Results. In the long-term period, residual changes (thickening) in the pleural cavity were found in 6.8% of patients in the main group vs. 19.8% of patients in the comparison group (p < 0.05), changes in diaphragmatic skeletopy were observed in 13.5% vs. 25.9%, respectively (p < 0.05), and diaphragmatic mobility disorder – in 5.4% vs. 18.5%, respectively (p < 0.05). Patients with tuberculous pleurisy who had been examined according to the proposed algorithm developed chronic pleurisy 7.1 times less often (1.4% vs. 9.9%, p < 0.05) and pulmonary disease – 2.7 times less often (4.1% vs. 11.1%, respectively, p ˃ 0.05).\u0000Conclusion. The study involved students of 18–20 y/o, I–II course of the medical university. According to the results of screening of 448 students, we used a standardized questionnaire by OM Vein. There were 82 people with signs of autonomic dysfunction, which comprised the main study group. To assess the state of autonomic balance, we used cardiointervalography; to verify the hyperventilation syndrome, we used the Nijmegen questionnaire; to assess the psycho-emotional state and anxiety, we used the State-Trait Anxiety Inventory.\u0000Results of the research. Among 82 students with signs of autonomic dysfunction, the average score by the Vein scale was 36.12 ± 4.26 points (p ˂ 0.001) vs. 15 points denoting the norm. Subjective symptoms identified by the Vein scale closely correlated with the results of the Nijmegen questionnaire. A positive indicator of the latter – 36.24 ± 1.26 points – was observed in 71.95 % of people, which indicated the presence of hyperventilation syndrome in this group of students. The rest of the students in the main group had a negative result (less than 23 points), although according to the Vein scale, they had signs of autonomic dysfunction, which could indicate a certain premorbid condition. According to the State-Trait Anxiety Inventory, among the 82 people surveyed, students with signs of personal anxiety scored the most points, which were accompanied by emotional \"explosions\" (56.1 %), neurotic conflicts (35.37 %), feelings of insecurity (46.34 %) and anxiety (37.8 %), etc. We found an association between signs of anxiety and hyperventilation syndrome (r = 0.76). According to the results of the assessment, against the background of anxiety and neurotic symptoms and manifestations of hyperventilation syndrome, the results of the survey revealed signs of somatoform autonomic dysfunction in a significant proportion of students (71.95 %) of the main group, including complaints of respiratory, circulatory and gastrointestinal nature. The lack of isolated psychological markers specific to each psychosomatic disorder should also be noted. Thus, the identified signs of autonomic imbalance by the Vein questionnaire were significantly correlated with the manifestations of hyperventilation syndrome, increased anxiety, somatoform abnormalities, which is the evidence of statistical significance and informativeness of the results of the surveys.\u0000Conclusions. However, the exact mechanisms of connection between inflammation and airway remodeling are not very well understood and that is the reason for delay of development new specific targeted drugs aimed to inhibit the process of inflammation and remodeling. Therefore, our goal was aimed to analyze and systematize data on the role of matrix metalloproteinases in the occurrence of airway remodeling in patients with bronchial asthma and the possibility of therapeutic effects on this process.\u0000Materials and methods: search for information on the role and mechanisms of influence of matrix metalloproteinases on airway remodeling processes in patients with bronchial asthma and the possibility of its pharmacological correction in electronic databases such as PubMed and Google Scholar over the past 25 years.\u0000Matrix metalloproteinase-9 and tissue proteinase-1 inhibitor have been shown to play the most important role in airway remodeling in the presence of bronchial asthma, supported by numerous experimental and clinical studies. Much attention is paid to the comparison of these indicators in bronchoalveolar lavage, induced sputum and blood on the background of exacerbation and in the presence of a stable course of the disease. The analysis of their content depending on the severity of the course, dysfunction of external respiration and the degree of reversibility of bronchial obstruction. Studies of the possibility of drug effects on the content of remodeling markers have shown low clinical efficacy. Dys + phagein – eat, swallow) – is a violation of swallowing, in which there is a feeling of discomfort or difficulty in transporting the food bolus from the mouth to the stomach. Because the disorder of the act of swallowing in stroke is usually of neurogenic origin and is defined by a number of neurologic disturbances and dysfunctions, it is better to use the term \"neurogenic dysphagia\" for patients with acute disturbance of cerebral circulation. The urgency of this problem is especially emphasized in the development of a set of therapeutic exercises for such socially important disease as acute cerebrovascular accident. One of the formidable complications of stroke is a violation of the act of swallowing. This leads to a significant reduction in quality of life increasing the risk of secondary complications, which, in turn, significantly increases the likelihood of death.\u0000Violation of the act of swallowing is observed in 26–45% of patients due to acute ischemic stroke. Dysphagia leads to the development of life-threatening complications such as aspiration pneumonia, dehydration, and significantly reduces the quality of life of such patients. Thus, the correction of the act of swallowing by means of physical rehabilitation after brain injury is a very important task of the rehabilitologist. For a long time, speech therapy was the only method of rehabilitation of dysphagia; now we can widely use therapeutic exercises to restore the functions of facial muscles and masticatory muscles.\u0000Screening for swallowing disorders allows us to assess the function of the masticatory muscles in five stages, which makes it possible to develop a more individual and thorough set of therapeutic exercises after each stage of the screening study. They were divided into 3 groups: the first group consisted of 182 children with arterial hypertension, the second group – 78 children with prehypertension and the control group – 588 children with normal blood pressure level. The physical development of children was assessed according to the generally accepted unified method. The blood pressure level was measured with an automatic tonometer three times, with the calculation of the average value. The results were assessed using percentile tables. Statistical processing of the data obtained was carried using Microsoft Office Excel and Statistica 10.0 programs.\u0000When analyzing anthropometric indicators, it was found that the Z-index of the body mass index in children of the first group was significantly higher than that in children with normal blood pressure. There was a difference in the circumference of the waist and hips as compared with children from the second and control groups.
