Medical advances, particularly through minimally invasive procedures, make numerous interventions possible even for older people. This is leading to a current increase in medical demand that is expected to continue into the future. Declining birth rates have led to a shortage of skilled workers in the healthcare sector, which is projected to be even more pronounced in coming years. This is leading to a staff shortage, followed by increasing workloads. Shift work, heavy bureaucratic burdens, and inflexible work hours are making patient-centered careers increasingly unpopular. Although many young people can imagine working in nursing or medical professions, the current working conditions mean that many young professionals prioritize private and family life, seek part-time jobs, or leave the profession altogether. Only improvements in working conditions and, where appropriate, adjusted remuneration can reverse this trend. Reduced bureaucracy, predictable working hours and vacation times, as well as modern personnel development and leadership are potential solutions. Early diagnosis and appropriate treatment are crucial for the prognosis and quality of life of affected children.</p><p><strong>Objective: </strong>The aim of this article is to provide an overview of the etiology, classification, clinical presentation, diagnostic tests, and treatment of pediatric testicular tumors.</p><p><strong>Materials and methods: </strong>This article is based on an analysis of current literature.</p><p><strong>Results: </strong>Pediatric testicular tumors are benign in 60-75% of cases, most commonly teratomas or Leydig cell tumors. Malignant tumors such as yolk sac tumors are rare and show low metastatic potential. Diagnostics include ultrasound, tumor markers (AFP, hCG, LDH), and imaging (MRI, CT). Treatment consists of organ-sparing surgery in suspected benign cases and orchiectomy in malignant cases. Chemotherapy is reserved for metastatic tumors. In localized stage I disease, surveillance is often sufficient.</p><p><strong>Conclusion: </strong>Pediatric germ cell tumors differ from adult testicular tumors in terms of incidence, malignancy, and histology. A major goal of residency programs in urology is the teaching of practical skills in interventional and conservative urology so that residents will be able to implement those skills independently as board certified urologists. Residency programs in Germany, however, are highly heterogeneous due to the fact that the specific contents are defined by state (Länder) medical associations. Furthermore, the implementation of the specific contents is the responsibility of the program directors, which is not assessed transparently. This inconsistency might explain the fact that about one third of residents are highly dissatisfied with the quality of their training programs. To meet future challenges due to the aging population and the already existing additional need of well-trained urologists, we need (1) to implement nationwide, structured, uniform, generally binding and transparent residency programs, (2) the teaching of communicative skills with patients and within interdisciplinary teams, and (3) work schedules that are flexible, including interdisciplinary education, regional and supraregional rotations. In a modular system, the program includes rotations in the outpatient area and optionally in other specialist disciplines or other clinics with different focuses. Integrated into the training period are 5 module seminars and a series of webinars with regular learning success checks, in which the entire learning content listed in the training regulations is presented in interactive presentations.
