The main cause of CVD is the formation of thrombi due to by unstable atherosclerotic plaque rupture on the arterial wall. Long-term accumulation of thrombi results in vascular remodeling, and subsequent-stenosis of the lumen obstructs the blood flow, thereby leading to myocardial tissue ischemia and hypoxia. Sustained ischemia and hypoxia lead to myocyte necrosis, resulting in irreversible myocardial injury. Many molecular and cellular mechanisms are associated with atherosclerotic plaque instability (API). For example, macrophages can produce various inflammatory factors, adhesion factors, chemokines and matrix metalloproteinases (MMPs), which play important roles in the pathophysiological mechanisms of API and in maintaining plaque stability. These molecules may help predict unstable atherosclerotic plaques. If the plaque is stable, it will not be prone to rupture or thrombosis. Accordingly, in this review, we will discuss the different pathophysiological mechanisms of API and the related roles of macrophages in the mechanisms of API mainly in animal models and humans. {"title":"The Effect of HIF1-α/NF-κB/MMP2 Signaling Pathways Mediated by ALDH2 on Chronic Intermittent Hypoxia Induced Myocardial Injury in Rats","authors":"Bin Chen","doi":"10.54097/wxcz8y41","DOIUrl":"https://doi.org/10.54097/wxcz8y41","url":null,"abstract":" (1) Objective: To investigate the effect of HIF1-α/NF-κB/MMP2 signaling pathways mediated by ALDH2 on chronic intermittent hypoxia induced myocardial injury in rats. (2) Methods: Thirty SD rats were randomly assigned to three groups (10 rats in each group): control group (CONTROL), chronic intermittent hypoxia group (CIH), and CIH + ALDH2 group. Rats in the CIH + ALDH2 group were established by an intraperitoneal injection of the ALDH2 activator, Alda-1 (20 mg/kg), once a day for three consecutive days. After three months, changes in ventricular function were determined by ultrasound. The expressions of HIF1-α, NF-κB, and MMP2 proteins were detected by protein blotting, and the concentration of 4-HNE in myocardial tissue was measured. (3) Results: Compared with the control group, echocardiography showed that rats in the CIH group had significantly reduced myocardial function, elevated protein levels of HIF1-α, NF-κB, and MMP2 in myocardial tissues, and increased the expression of 4-HNE in myocardial tissues (P<0.01); Compared with the CIH group, rats in the CIH + ALDH2 group showed significant improvement in myocardial function, decreased protein levels of HIF1-α, NF-κB, and MMP2 in myocardial tissue, and decreased the expression of 4-HNE in myocardial tissue (P<0.01). From June 2022 to December 2022, 90 patients receiving MHD treatment in Blood Purification Center of XXX Hospital were randomly divided into observation group and controls, with 45 patients in each group. The controls received routine hemodialysis care, and observation group received resistance exercise with moderate and low inte in addition to routine hemodialysis care. QoL measurement tool adopted 36-Item Short Form Health Survey (SF-36), Hamilton Rating Scale for Depression (HAMD), and Hamilton Anxiety Scale (HAMA) to evaluate patients. The physical function was 67.64±25.62 and mental health was 71.68±28.55 in controls, and those was 71.11±25.42 and 73.48±13.59 in observation group; in terms of physical function and mental health in QoL, observation group was obviously higher; and HAMD and HAMA scores in observation group were clearly lower as against controls (P < 0.05). The effect of resistance exercise with moderate and low inte was obvious on the physical function and mental state of QoL in MHD patients, but not obvious on other aspects. The discovery and development of targeted therapies, such as the tyrosine kinase inhibitor STI-571 (Imatinib), revolutionized the treatment of CML by specifically inhibiting the BCR-ABL kinase activity. This abstract provides a summary of the therapeutic efficacy and clinical impact of STI-571 in the management of CML.Sti-571, a specific tyrosine kinase inhibitor, is a novel antitumor drug that has entered clinical trials and can effectively treat chronic myeloid leukemia caused by abnormal proliferation of hematopoietic stem cells. In this paper, the mechanism of action and regulation of cell cycle of this drug were summarized, and the problems of drug resistance were explored in depth and the development prospects of this drug were briefly introduced. The introduction of STI-571 has dramatically changed the management of CML, leading to a paradigm shift from conventional chemotherapy and allogeneic stem cell transplantation as the primary treatment options. The drug has shown superior efficacy compared to interferon-alpha therapy, the standard treatment before its introduction. STI-571 is generally well-tolerated, with fewer severe adverse effects, making it an attractive option for long-term therapy. Resistance to STI-571 has been observed in a subset of patients, primarily due to the emergence of BCR-ABL kinase domain mutations. Methods: The data of 2 patients with late-onset myasthenia gravis were collected, including basic information, clinical features, auxiliary examinations, treatment and effects. Results:: Both male patients were elderly. The clinical manifestations were bulbar palsy with diplopia or limb weakness. Symptoms were mild in the morning and severe in the evening, and the neostigmine test was positive. According to the criteria, myasthenia gravis was diagnosed, and pyridostigmine treatment was effective. The severity of the condition was different. Tests for myasthenia gravis antibodies, thyroid function, autoimmune antibodies, and tumor markers were also different. Follow-up of treatment effect was required for prognosis. Conclusion: Late-onset myasthenia gravis is easily misdiagnosed. Neurological examination can help to detect skeletal muscle involvement, and medical history can help to detect clinical features.
