{"title":"Quality of Sleep and Related Factors on Elderly Residents of the Nursing Homes","authors":"N. Yurt, Mahcube Cubukcu","doi":"10.18521/KTD.815000","DOIUrl":"https://doi.org/10.18521/KTD.815000","url":null,"abstract":"Objective: Our study aims to determine the rate of sleep disorders in elderly people aged 65 and older, living in nursing homes in Samsun, and also to investigate the factors affecting sleep quality through a comprehensive geriatric assessment. Methods: Our study was performed in the two largest nursing homes in Samsun province between March 1st and April 30th 2018. The sleep quality of the participating individuals was assessed by the Richard’s Campbell Sleep Questionnaire (RCSQ) and the Epworth Sleepiness Scale (ESS). This individuals in our study were assessed using Geriatric Depression Scale (GDS), Mini-Mental State Examination (MMSE), Short Form 36 Health Survey (SF-36), Charlson Comorbidity Index (CCI). The obtained data were analyzed with SPSS 23.0 and p<0.05 was considered as significant. Results: A total of 84 people were included in the study and the mean age was 77.06±7.97 years. It was determined that 23.8% of the individuals had poor sleep quality at night according to RCSQ and 42.9% of the individuals had increased daytime sleepiness according to ESS. A significant statistical difference was found between the GDS scores with the RCSQ and ESS scores (p<0.001, p=0.001, respectively). The presence of depressive symptoms was associated with sleep quality at night and daytime sleepiness. A statistically significant difference was found between the MMSE and RCSQ scores (p=0.014). The presence of dementia was associated with sleep quality at night. Conclusions: The presence of depressive symptoms was found to cause poor sleep quality at night and increased daytime sleepiness. People with dementia had low sleep quality at night. Metod: Bu calisma tek merkezli retrospektif olarak yapildi. Calismaya 01 Ocak 2019- 31 Aralik 2020 acil servise basvuran 18 yas uzerindeki 1645 adli vaka dahil edildi. Calisma icin oncesinde Lokal Klinik Arastirmalar Etik Kurulu’ndan etik onay alindi. 11 Mart 2020- 31 Aralik 2020 tarihleri arasi COVID-19 pandemisi (DP), onceki tarihler ise pandemi oncesi (BP) olarak kabul edildi. BP ve DP donemlerinde adli vakalarin dagilim oranlari karsilastirilmistir. Bulgular: Trafik kazalari ve is kazalari erkek cinsiyette kadinlara gore yuksek saptandi, buna karsilik kadinlarda intihar girisimleri erkeklerden yuksek saptandi (p <0,001). Hem kadinlarda (BP: %14,9, DP: %17,4) hem de erkeklerde (BP: %3,7, DP: %4,9) intihar girisimlerinin artis gosterdigi saptandi (p <0,01). Pandemi sirasinda, pandemi oncesiyle karsilastirildiginda kadinlarda (BP: %22,3, DP: %13,4) ve erkeklerde (BP: %22,5, DP: %21,1) is kazalari dusuk saptandi (p <0,01). Sonuc: COVID-19 pandemisi hem kadinlarda hem de erkeklerde intihar oranlarini arttirmis, is kazasi oranlarini ise azaltmistir. Kadinlardaki intihar oranlari erkeklerden yuksek bulunmustur. Ozcelik","doi":"10.18521/KTD.767493","DOIUrl":"https://doi.org/10.18521/KTD.767493","url":null,"abstract":"Objective: To investigate the emergency department admissions, the general surgery consultation request, and the necessity of immediate treatment of patients despite the fact that the society did not leave the house unless necessary during the pandemic period. Methods: The files of patients who admitted to the emergency department and whose general surgery consultation was requested due to urgent surgical treatment were retrospectively scanned between March-May 2020, which is the Covid 19 pandemic period and March-May 2019, as a time period in which normal social life continues. Age, gender, complaints, comorbid diseases, the necessity of truly emergency surgical treatment as well as the socioeconomic level scale for each individual were recorded and analyzed. Results: The rate of request for consultation was 2.7% (n = 170) in the pandemic period, and 3.1% (n = 316) in the non-pandemic period. In both periods, no statistically significant difference was found in the distribution of genders according to the treatment(p> 0.05). The complaints of the patients were similar in both periods, and the most common complaints were nausea-abdominal pain-difficulty defecation. Although individuals without comorbidities are predominant in both periods, the proportion of individuals with comorbid diseases was higher during the pandemic period (p = 0.001). Patients who did not require urgent treatment in both periods were in the majority. In patients requiring urgent treatment, the rate of the hospitalized patients was higher during the pandemic period, whereas the rate of the operated patients was higher in the non-pandemic period (p = 0.005). The majority of the patients had a low socioeconomic level and a small portion had a moderate socioeconomic level in both periods (p> 0.05). In addition, patients with a low socioeconomic level and 75 years of age or older who did not require urgent treatment were higher in the pandemic period (p = 0.002). In this study, hypnotherapy was applied to two patients to whom dental procedures could not be carried out due to gagging. The first case was one in which it was thought that the patient would be unable to use dentures due to gagging, and, after hypnotherapy, it was ensured that she was able to use her dentures without any problem. The second case was a hypnotic-resistant case, and the impression procedure was carried out only after hypnotherapeutic suggestion. Genel ve lokal anestezi altinda yapilan septorinoplasti operasyonlari sirasinda trigeminokardiyak refleks gelisebildigi gozlenmistir. Retrospektif calismamizda genel anestezi altinda yapilan septorinoplasti operasyonlarinda birincil olarak cerrahi olarak refleksin uyarildigi dusunulen periyodlarda trigeminokardiyak refleks gelisimi arastirilmistir. Ikincil olarak da farkli inhalasyon anestezik ajanlarin bu refleksin ortaya cikisinda etkisi arastirilmistir. Yontem: 01/ Ocak/2016- 30/Kasim/2016 tarihleri arasinda opere edilmis septorinoplasti vakalarinin anestezi notlari ve Datex Ohmeda icentral merkezi monitor kayitlari ve MIA-MED Hastane Bilgi Yonetim Sistemi yazilimi uzerinden retrospektif olarak incelenerek saptandi. Induksiyon, lokal anestezi uygulamasi, cerrahi insizyon, insizyon suturasyonu baslangici ve ekstubasyondan 5 dakika sonra degerleri kayitlardan saptandi. Kriterlere uyan 60 hastada 2 farkli inhalasyon anestezik madde uygulandigi tespit edilerek grup sevofluran ve grup desfluran olarak 2 ayri grupta analizler yapildi ve QT, QTc ve Trigeminokardiyak refleks gelisimi arastirildi. Bulgular: Gruplar arasinda fark olmamasina ragmen bazal degerler ile diger periyodlar karsilastirildiginda TKR gelisiminin ve QT, QTc uzmanlarinin en fazla sinirli uyusturma uygulamasi, cerrahi insizyon, insizyon suturasyonu periyotlarinda izlendigi saptandi. Yontem: Retrospektif olarak 48 YAB tanili cocuk ergen ile yas-cinsiyet acisindan eslestirilmis 46 saglikli kontrol cocuk ve ergenin tam kan sayimi sonuclari; hemoglobin (Hb), eritrosit dagilim genisligi (RDW), trombosit sayisi (PLT), ortalama trombosit hacmi (MPV), trombosit dagilim genisligi (PDW), plateletkrit (PCT), beyaz kan hucre sayimi (WBC), notrofil, lenfosit ve monosit sayilari ve notrofil / lenfosit orani (NLR), monosit / lenfosit orani (MLR), trombosit / lenfosit oranlari (PLR) acisindan karsilastirildi. Istatistiksel degerlendirmede ki-kare testi, bagimsiz orneklemler t-testi ve Mann-Whitney U testi kullanildi. Bulgular: YAB olan cocuk ve ergenlerde, MPV duzeyleri anlamli olarak yuksek (p = 0,020) ve PLT ve PDW duzeyleri anlamli olarak dusuk (sirasiyla p = 0,018 ve p = 0,011) bulundu. Hb, RDW, PCT, WBC, notrofil, lenfosit ve monosit sayilari, NLR, MLR, PLR acisindan olgu ve kontrol gruplari arasinda istatistiksel olarak anlamli bir farklilik saptanmadi. Yontem: Bu calismaya 2009-2013 yillari arasinda izlenen 104 diyabetik hastanin (46 erkek hastanin 92 gozu, 58 kadin hastanin 115 gozu) toplam 207 gozu dahil edildi. PLSK, OCT ile ust, alt, temporal ve nazal kadranlarda olculdu ve demografik faktorlerle karsilastirildi. Bulgular: 60 yas ve uzerinde ortalama PSLK, 60 yas altina gore ust ve alt kadranda daha ince oldugu goruldu(p=0,004, p=0,001). Cinsiyet ile ortalama PSLK arasinda sadece alt kadranlarda anlamli bir iliski vardir (p = 0,006). Dort kadranda da hipertansiyon(HT) ile ortalama PSLK arasinda iliski yoktur(p>0.05). Glokomlu gozlerde, glokomu olmayanlara gore ust kadranda ortalama PLSK azalmaktadir(p=0.019). 15 yildan fazla diyabet suresi olan hastalarda ust, alt ve nazal kadranlarda ortalama PLSK, 15 yil veya daha az olanlara gore azaldigi goruldu(p=0,048, p=0,020, p=0,020). Retinopatisi olmayan gozlerde ortalama temporal PLSK, proliferatif diyabetik retinopatili (PDR) ve non-PDR olan gozlerle karsilastirildiginda azalmisti(p=0,025). Temporal PLSK hem diyabetik makula odemi (DMO) olan hem de panretinal fotokoagulasyon uygulanan hastalarda artmisti (p=0,001,p=0,001). Fokal lazer tedavisi uygulanan hastalarla dort kadrandaki ortalama PLSK arasinda korelasyon bulunmadi (p>0.05). Sonuc: Yas, cinsiyet, diyabet suresi, glokom varligi, DMO, PDR-nonPDR gibi faktorlerin PSLK uzerine etkisinin oldugu fakat HT, fokal lazer gibi faktorlerin PSLK uzerine etkisi yoktur.
