In autoimmune thyroiditis, in addition to impaired immunological tolerance, a significant role is played by biochemical markers such as matrix metalloproteinases and antibodies to deoxyribonucleic acid (DNA), which can contribute to the pathogenesis of the disease. \u0000Aim. Studying the activity of metalloproteinases-3, -7 and the level of antibodies to DNA in patients with autoimmune thyroiditis. \u0000Material and methods. In 2019–2021 a non-randomized study in the research laboratory at the Department of Biochemistry of the Azerbaijan Medical University, during which the blood serum of 170 patients with autoimmune thyroiditis, 64 (37.6%) men, average age 39.4±11.2 years and 106 (62.4%) women, average age 37.8±9.05 years, was conducted. Inclusion criteria: men and women aged 18 years and older with a primary diagnosis of “autoimmune thyroiditis” without concomitant allergic or other autoimmune diseases and severe somatic disorders. The control group consisted of 65 people (average age 38.7±10.8 years) without any thyroid disease or autoimmune pathology. Analysis of clinical and laboratory studies with determination of the level of thyroid-stimulating hormone, free thyroxine and free triiodothyronine made it possible to divide patients into two groups: the first group — 74 patients with a subclinical form of the disease; the second group — 96 patients with a manifest form of the disease. In all patients, the activity of metalloproteinases-3 and -7 in the blood serum was determined by enzyme-linked immunosorbent assay, and the content of antibodies to native and denatured DNA was determined by enzyme-linked immunosorbent assay. Intergroup comparisons on quantitative indicators were carried out using the nonparametric Mann–Whitney rank test, taking into account the difference in the distribution of the analyzed indicators from the normal one. Qualitative characteristics were assessed as percentages. \u0000Results. The median activity of metalloproteinases-3 and -7 in the total group of patients was 56 (51.0; 59.0) and 4.6 (4.3; 5.2) ng/ml, respectively, which was significantly higher than the control values — 23.0 (16.0; 26.0) and 3.6 (3.4; 4.1) ng/ml, respectively (p=0.0001252 and p=0.00054). There were no statistically significant differences in the level of metalloproteinase-7 in patients with subclinical and manifest thyroiditis (p=0.097), while the median activity of metalloproteinase-3 in the manifest group was 59.0 (56.0; 65.0) ng/ml and it was higher than in patients of the subclinical group by 13.2%. The content of antibodies to native DNA in patients was 4.4 (2.3; 9.9) units/ml, which was statistically significantly higher compared to the control group — 2.6 (1.5; 3.6) units/ml (p 0.001), while no significant differences were found in the levels of antibodies to denatured DNA in patients compared to control values. \u0000Conclusion. The new coronavirus infection COVID-19 is a pressing health problem around the world. This disease affects the respiratory, cardiovascular and other systems. The role of serotonin as one of the key factors in COVID-19 is discussed in the literature. \u0000Aim. Determination of the concentration of serotonin and its metabolite 5-hydroxyindoleacetic acid in the blood serum of children in the acute period of COVID-19 for use as a diagnostic criterion for the severity of the disease. \u0000 \u0000Material and methods. A study that included 141 children aged 0 to 17 years was conducted: 92 patients hospitalized in a hospital with a diagnosis of COVID-19 without concomitant pathology made up the study group, 49 healthy children were included in the control group. The subjects were divided into two age groups: younger (0–2 years) and older (3–17 years). The groups of patients and children from the control group were comparable in age and gender. A study of the concentration of serotonin and its metabolite — 5-hydroxyindoleacetic acid — in the blood was carried out using high-performance liquid chromatography, and the results of laboratory and instrumental research methods (obtained from medical records) were evaluated. Based on the results of computed tomography, the proportion of involvement of the lung parenchyma in the pathological process was calculated: CT0 — absence of lung damage; CT1 — less than 25%; CT2 — 25–49%; CT3 — 50–74%; CT4 — more than 75%. Quantitative data were described using median (Me) and lower and upper quartiles (Q1–Q3). To analyze the data, the Pearson χ2 test, nonparametric Kruskal–Wallis and Mann–Whitney tests, as well as the ROC curve method were used. A correlation analysis was carried out using the Spearman coefficient and an assessment of the dependence of a quantitative variable on factors using the linear regression method. \u0000Results. In children with COVID-19 aged 0–2 years, the concentration of serotonin (969.41 pmol/ml) in the blood serum was statistically significantly higher by 49.1 times, in the group of 3–17 years (637.87 pmol/ml) — by 24.5 times compared to age-matched controls (19.74 pmol/ml; p 0.001 and 26.0 pmol/ml; p 0.001, respectively). The concentration of 5-hydroxyindoleacetic acid in patients 0–2 years old (204.90 pmol/ml) was 4.7 times higher relative to healthy children (43.56 pmol/ml; p=0.003), and in the older age group (78.25 pmol/ml) — 2.4 times higher (32.65 pmol/ml; p=0.012), which suggests increased breakdown of serotonin under the influence of monoamine oxidase A. Maximum concentrations of serotonin in blood serum were detected in children aged 0–2 years (1808.79 pmol/ml) with damage to the lung parenchyma from 50 to 74% (grade 3 according to computed tomography). However, in children aged 3–17 years, the maximum concentrations of serotonin (2036.95 pmol/ml) were detected with lung damage ranging from 25 to 49% (grade 2 according to computed tomography). It has been established that there are no functional differences between the paired salivary glands, but the presence or absence of structural differences in the contralateral salivary glands remains unknown. Aim. To determine the presence of features of the contralateral salivary glands’ structural organization, which should be taken into account in the process of differential diagnosis in patients with various diseases of the salivary glands when analyzing sialograms. Material and methods. In 60 patients with diseases of the salivary glands, digital subtraction sialography of the parotid (right n=33, left n=26) and submandibular (right n=17, left n=19) glands was performed. The parameters of the paired glands were compared: the number and time of filling of the ducts with the water-soluble non-ionic contrast agent iodixanol. The sample was formed without taking into account gender, age and diseases. Cases with stones or ductal strictures were excluded. The significance of differences was assessed using Student's t-test. The results were considered significant at p ≤0.05. Results. The contrast time for the parotid glands (n=59) averaged 29.8±8.9 s: on the right — 31.0±8.0 s (n=33), on the left — 28.2±9.6 s (n= 26; t=1.2219); submandibular glands (n=36) — 28.6±11.6 s: on the right — 30.2±14.4 s (n=19), on the left — 27.3±8.3 s (n=17; t= 0.7284). The amount of drug injected into the ducts reflected their volume, which we considered as an accurate characteristic of their structural state. 1.4±0.3 ml of iodixanol (Visipack) was injected into the parotid glands (n=59): on the right — 1.4±0.3 ml (n=33), on the left — 1.4±0.3 ml (n =26; t=0). 1.2±0.3 ml was injected into the submandibular glands (n=35): on the right — 1.2±0.4 ml (n=17), on the left — 1.1±0.3 ml (n=18; t=0.8399). There was no significant difference between the contralateral glands, but there was a significant diffe¬rence between the parotid and submandibular glands (t=3.1247; p ≤0.001). When conducting scientific analysis, data from the contralateral salivary glands could be combined into a single sample. The amount of drug injected into the ducts served as a characteristic of the structural state of the gland. Conclusion. When using electrical muscle stimulation in clinical practice, it is important to select the optimal mode of this effect, since muscles largely determine the movement of the limb during the rehabilitation period. Aim. Study of the tibialis anterior muscle reaction during Ilizarov distraction of the tibia in combination with the direct effect of direct electric current on the regenerated area in the experiment. Material and methods. The tibialis anterior muscle and biochemical parameters of blood serum (creatin kinase activity, lactate concentration) of 27 male Soviet chinchilla rabbits aged 12 months, weighing 3.85±0.18 kg, tibia length 11.2±0.13 cm, were studied. The animals were divided into three groups: control (n=9), first (n=9) and second experimental (n=9). The right tibia was fixed with an Ilizarov apparatus, a transverse osteotomy was performed in the middle third of the diaphysis, and from the 5th day, distraction began at a rhythm of 0.125 mm in 4 steps to an amount of 10% of the original length for 26 days. Fixation lasted 40 days, the period without the device was 30 days. For electrical stimulation, wire-electrodes were inserted into the diaphysis, and electrical stimulation of the bone regenerate was performed for 1 minute with a current intensity of 150 mAm. In the first group, electrical stimulation was performed starting from the day of surgery and on days 2, 4, 6, 8, 10 of the experiment. In group 2, electrical stimulation began on the 10th day after surgery and on the 12th, 14th, 16th, 18th, and 20th days of the experiment. In the control group, no electrical stimulation was applied. Using the methods of stereometric analysis of digitized images of tibialis muscle’s cross sections, the volumetric density of myosymplasts, microvessels, endomysium and nuclear component, the numerical density of myosymplasts and microvessels were determined, and the vascularization index was calculated. For statistical processing of data, the Wilcoxon W test and the Mann–Whitney T test were used; numerical data were presented in tables. Results. A positive effect of electrical stimulation on the muscles of the experimental groups was established in comparison with the control group, where fibrosis of muscle tissue at the end of the experiment was 0.2777±0.0055 mm3/mm3, which was 230% relative to the parameter of the first group (0.1217±0.0121 mm3/mm3) and 370% relative to the second group (0.0752±0.0062 mm3/mm3). An advantage was noted for the second group, where electrical stimulation was carried out from the 5th day of distraction and at the end of the experiment the histostructure of the muscle, characteristic of the intact norm, prevailed. Conclusion. Upper thoracic aperture syndrome occurs in 2–8% of people in the population and affects patients of the most working age, mainly women. Aim. Evaluation of the combined (drug + osteopathic) treatment effectiveness of patients with upper thoracic aperture syndrome compared to drug therapy. Material and methods. In a randomized study, patients with signs of upper thoracic aperture syndrome were divided into two groups: the main group received combined (osteopathic + drug) treatment for 2 months; the control group received only drug treatment (non-steroidal anti-inflammatory drugs, muscle relaxants, thioctic acid preparations) for 2 months. Each group consisted of 15 women aged from 25 to 47 years (main group — 36.31±7.33 years, control group — 35.18±6.76 years), the duration of the disease ranged from 6 to 18 months (main group — 12.02±3.57 months, control group — 11.07±3.97 months). The groups were comparable in age composition and duration of the disease. To assess the effectiveness of the treatment, the dynamics of symptoms of neuropathic pain, stimulation electroneuromyography, and the dynamics of osteopathic status indicators were used. Data were presented as arithmetic mean and standard deviation. When analyzing intergroup differences, the nonparametric Mann–Whitney test (U-test), and when analyzing intragroup differences, the nonparametric Wilcoxon test (W-test) were used. The statistical significance of the difference in the frequency of osteopathic dysfunctions and other frequency parameters before and after treatment was assessed using the Pearson χ2 test. The critical level of statistical significance was taken as 5% (p=0.05). Results. The combination of osteopathic and drug treatment led to a difference in conduction along the right ulnar nerve between groups (p=0.0003) in the main group — an increase in speed from 48.1±1.54 m/s to 61.0±3.34 m/s (p=0.0007), in the control group — from 46.8±1.89 m/s to 50.2±4.85 m/s (p=0.0009). Analysis of the NTTSS-9 scale (assessment of symptoms of neuropathic pain) after treatment showed no difference between the groups (p=0.0941): the main group — before treatment 14±2.14 points, after treatment 2±1.4 points (p =0.0003), control group — before treatment 15±1.95 points, after treatment 6±2.80 points (p=0.0007). Conclusion. Professor P.M. Argutinsky-Dolgorukov connected mainly with the study of infectious pathology. His students Professor V.K. Menshikov, E.M. Lepsky, A.A. Agafonov later became well-known pediatricians and infectious disease specialists who took an active part in the creation of pediatric and infectious diseases services in the Republic of Tatarstan. P.M. Argutinsky-Dolgorukov was the founder of the study of malaria in children in Russia. To give a comprehensive description of the dynamics of special indicators of population aging and to assess the demographic aging scale of the population of the Irkutsk region. \u0000Material and methods. A comprehensive assessment of the dynamics of population aging was carried out using 6 special indicators of population aging and 3 indicators of demographic movement. The calculation of the values of the indicators was performed in accordance with the methodological provisions on demographic statistics. The scale of aging was calculated based on the results of determining the stages of aging using 4 parameters of population aging: the level of aging, the depth of aging, aging factors, and the rate of aging. The data source was the statistical accounting tables of Rosstat for the Irkutsk region: А12а_25 natural population movement; t_pt1n general results of population migration by incoming territories; 25_2PH population by sex and age. The number of statistical accounting tables separately for each municipality of the Irkutsk region and the region as a whole amounted to 99 units for each year under study. \u0000Results. The dynamics of the aging level parameter indicates an aggravation of the aging processes of the population, and not due to an increase in people at the age of longevity (from 75 to 90 years) and centenarians (90 years and older), but due to the population older than working age, women from 55 years old and men from 60 years up to the age of 75 years. Indicators of aging factors that characterize reproductive and migration compensatory mechanisms are at a level that does not allow compensating for the growing processes of population aging. The value of the aging rate parameter in terms of the growth rate of the aging coefficient in dynamics indicates the stability of the demographic aging of the population. An analysis of the scale of population aging in the Irkutsk region and its districts showed that the Irkutsk region has an average value of parameters and is at an intermediate stage between the aging and old population. \u0000Conclusion. The study of the influence mechanisms of the SARS-CoV-2 virus on human homeostasis is still relevant. Of particular interest is the study of pathomorphological changes in the appendix in children with COVID-19 with the determination of the CD-phenotypic status of immunocompetent cells. \u0000Aim. Immunohistochemical evaluation of appendix inflammation in children diagnosed with COVID-19. \u0000Material and methods. The groups were formed on the basis of anamnestic, clinical and morphological data: the first group (n=42; age from 2 to 18 years, mean 10.84.9 years) surgical material of the vermiform processes of children with an established clinical diagnosis of coronavirus infection (COVID-19 based on PCR results); the second group (n=55; age from 2 to 18 years, mean 9.74.2 years) surgical material of the appendix after appendectomy in children with an established clinical diagnosis of acute appendicitis obtained before the onset of the COVID-19 pandemic; the third group (n=38; age from 2 to 18 years, mean 10.33.2 years) control group, autopsy material of appendixes (intact). Histological and immunohistochemical studies were carried out using primary antibodies to CD3, CD4, CD68, CD20, CD138. The number of CD+ cells was determined by computer morphometry in 10 fields of view. Student's t-test was used in quantitative analysis for a significant comparison between groups. Then, the quantitative density of CD+ cells per 1 mm2 was converted into a scoring system for visual presentation by a semi-quantitative method. \u0000Results. Most samples (n=41) of the first group showed signs of destructive phlegmonous-ulcerative appendicitis. An immunohistochemical study revealed an increase in the number of T-lymphocytes (CD3+, CD4+), macrophages (CD68+), B-lymphocytes (CD20+) and plasma cells (CD138+) in the mucous membrane of the vermiform processes of children of the first group. In children of the second group, all clinical and morphological forms of acute appendicitis were found, the phenotype of immunocompetent cells corresponded to bacterial inflammation. \u0000Conclusion. V.S.Gruzdev, having mastered under the guidance of Professor V.A.Manassein the first skills of scientific research, widely used them in his development as an obstetrician-gynecologist. Later he formed his own scientific school, which still exists today. One of his early works was the study of artificial insemination in mammals. This research, ahead of its time, marked the beginning of the study of in vitro fertilization. He revealed an important regularity in the development of a fertilized egg when it is planted in the oviduct. Other contemporary embryologists also spoke about this feature, mentioning that V.S.Gruzdev became the first scientist who described it. It is important that Viktorin Sergeevich proposed a plan to resolve the problem of infertile marriage, through his experiments as a result of his research, followers were able to significantly increase the number of pregnancies that occurred as a result of the treatment of obstruction of the fallopian tubes and identify problems associated with the pathology of spermatogenesis in men, and their role in an infertile marriage.
