{"title":"COMPARISON OF PHYSICAL DEVELOPMENT AND FITNESS INDICATORS OF FRESHMEN WITH DIFFERENT HEALTH CONDITIONS WHO HAVE UNDERGONE COVID-19","authors":"Kotova N.V., Klochkova S.V., Kolupaev V.A.","doi":"10.26787/nydha-2618-8783-2023-8-2-37-44","DOIUrl":"https://doi.org/10.26787/nydha-2618-8783-2023-8-2-37-44","url":null,"abstract":"To study the \"post-covid syndrome,\" as a natural consequence of the pathological course of the new coronavirus infection \"covid-19,\" data from a retrospective analysis of the incidence and testing of motor abilities of freshmen in 2022 are presented. The incidence rate of covid-19 among 2022 freshmen was significantly higher than among 2021 freshmen and had two peaks in October 2021 and February 2022. The ratio of girls who had been ill and did not have covid-19 was significantly higher than in the group of boys. Among young men who had recovered from covid-19, 14.86% belonged to a special medical group, while among those who were not sick, only 4.27% belonged to a special medical group. In the group of girls who had had covid-19, 17.45% belonged to the special medical group, and 11.42% among those who were not sick. Among young men who had been ill and did not have covid-19, 68.92 and 68.29% of freshmen, respectively, had a normal level of body mass index (BMI); 13.51 and 14.63% − reduced BMI values, and 16.22 and 13.42% − high BMI values. Among girls, 64.98 and 69.84% of freshmen were characterized by normal BMI values; 21.52 and 19, 34% − reduced values, and 13.08 and 8.86% − high BMI. Comparison of physical fitness indicators of freshmen who had been ill and did not have covid-19 were performed in subgroups with comparable BMI values. In the group of boys and girls with normal body mass index values, there were no significant differences in physical development and physical fitness among students who had been ill and did not have covid-19. In boys and girls with an insufficient body mass index who had recovered from covid-19, the indicators of active orthostatic test were significantly worse than in those who were not sick. Over the past few years, there has been a sharp decline in vaccinated children. The increase in the number of unvaccinated children is probably due to parents' concerns about the alleged negative impact of vaccines on health, as well as the presence of false information disseminated by the anti-vaccination movement. The purpose of our study was to assess the attitude of parents to preventive vaccinations according to epidemic indications, to identify the main reasons for refusals to vaccinate children. An anonymous survey was conducted of 250 parents of preschool children attending the children's polyclinic department of the medical and diagnostic complex (LDC \"MedGard\"), medical and rehabilitation centers \"MedGard\" (LRC \"MedGard\") in Samara in the period from September 2022 to March 2023. The study analyzed the knowledge of parents about preventive vaccinations for epidemic indications (against meningococcal infection, influenza, human papillomavirus, chickenpox, tick-borne encephalitis virus, rotavirus, hepatitis A). The article presents data on the sources of obtaining information about vaccination from parents, about the side effects of previously administered vaccines in this category of children. The study revealed the relationship between the source of knowledge and knowledge about preventive vaccinations in respondents. It is concluded that people who receive knowledge directly and from reliable sources, i.e. from medical personnel, demonstrate a higher level of knowledge and are less likely to worry about vaccination compared to those who receive information from other sources. The aim of the study - assessment of quantitative and qualitative indicators of the rehabilitation bed fund in the context of a separate entity (Astrakhan region) for the period from 2019 to 2022. Materials and methods – the study analyzed indicators of the level of hospitalization, average annual bed occupancy, bed turnover, and average length of stay in a bed. Results and discussion – the increase of the rehabilitation bed potential observed during the study period and the increase in the level of provision of the population with rehabilitation beds in the region is aimed at achieving the goals, indicators and results of the state program of the Russian Federation \"Health Development\", approved by the Decree of the Government of the Russian Federation of December 26, 2017 No. 1640. In the context of the state policy aimed at improving the efficiency of the use of healthcare resources, the optimization of inpatient medical care for medical rehabilitation in the region is carried out against the backdrop of the formation of a three-level system for the provision of medical care for medical rehabilitation, the use of infrastructure and bed capacity of private healthcare institutions on the terms of public-private partnership, timely re-profiling of inefficiently functioning beds, reduction of bed downtime, active use of hospital-replacing technologies, retraining of medical personnel in new specialties, provided for by the staff standards of procedures for medical rehabilitation. Current data on the prevalence and incidence of CGD in children vary greatly from different sources, and therefore it is difficult to form strategies aimed at preventing CGD at an early age, which leads to an increase in the number of patients with late diagnoses, as well as a decrease in the quality of life of patients and their families, and has significant socio-economic consequences. The aim of the study was to assess the prevalence of CGD among children living in the large administrative center of the Central Federal District, the city of Yaroslavl. A retrospective analysis of the clinical, endoscopic and morphological status of the upper digestive tract in children with chronic digestive diseases examined in the conditions of the consultative and diagnostic center (CDC) of the Regional Children's Clinical Hospital (RCCH) for the period 2017-2021 was carried out. Based on the data obtained, the following statistical indicators were calculated: gender-age composition of patients with CGD, morbidity, prevalence for specific annual indicators and age groups. At the time of the retrospective analysis, 2,416 children were diagnosed with CGD. In 2021, the incidence was 4.92 per 1,000 children, and the prevalence was 20.61 per 1,000 children, these indicators change every year in waves. Our data on the prevalence of CGD in the city of Yaroslavl are underestimated and have a wide spread in different age categories. Systemic inflammation may be part of the pathogenesis of traumatic brain injury. Purpose of the study: Purpose of the study: Structural and functional characteristics of the experimental root of the left lung in systemic inflammation and traumatic brain injury. Materials and methods. To reproduce traumatic brain injury, a modified weight-drop model was used. Albino rats were studied experimental animals. To inflict injury, a device with a load (weight 200 g) was used, which falls from a height of 1 m onto the parietal region of the animal's head. For the study of experimental fever, the animals were injected intramuscularly with the pharmacopoeial form of LPS pyrogenal solution. Results. A morphological and histochemical study of the caudal lobar bronchus of the left lung of rats was carried out. The average height of the bronchial epithelium of the caudal lobar bronchus of the left lung most significantly and significantly decreases with a combination of traumatic brain injury and systemic inflammation. The bronchial epithelium retains its cilia. With the combination of SI and traumatic brain injury on the 6th day, argentophilia and an increase in the size of the nucleoli of the bronchial epithelium significantly increase. Argyrophilia of connective tissue fibers is revealed. Conclusion. With a combination of traumatic brain injury and systemic inflammation, the most informative changes in the content of mast cells are at the point of contact of the adventitial membrane of the caudal lobar bronchus of the left lung of the rat with the adventitial membrane of the caudal lobar vein of the left lung. Mastocytes localized at the site of contact between the adventitial membranes of lobe bronchus and lobe vien remain an insufficiently studied cell population of this differ on in both humans an rat. Due to constantly increasing amount of data, healthcare systems around the world are facing challenges associated with data processing and analysis while keeping costs under control. There is a number of examples where the use of big data in healthcare already provides solutions that optimize patient care and generate value for healthcare institutions. However, a further increase in the amount and variety of dynamically changing data in healthcare systems requires that all the relevant stakeholders collaborate and adapt the design and performance of their systems. To this end, it is necessary both to invest in the human capital and to build the technological infrastructure to house and converge a huge volume of healthcare data. It is also important to provide a set of tools that can improve data analytics by creating interactive visual interfaces to help analysts navigate and make sense of massive datasets. Here we provide an overview of international advanced initiatives related to big data analytics in various sectors of public healthcare that are aimed at obtaining new knowledge, improving clinical care and rationalizing epidemiological surveillance. Here we also share our own experience of applying visual analytics tools in the \"Electronic Passport of Pediatric Oncology and Hematology Service.\" This passport was created by specialists from the Dmitry Rogachev National Medical Research Center of Pediatric Hematology, Oncology and Immunology of Ministry of Healthcare of Russia in order to improve the quality of specialized care in the Russian regions. This software solution is based on data from checklists and questionnaires (baseline assessment) completed during an outreach event and its preparation as well as from correspondence regarding the implemented corrective measures (baseline and follow-up assessments). This system is an important tool that helps improve the quality of monitoring of the pediatric oncology and hematology service. The structured electronic database allows the user to rank the Russian regions based on a whole range of parameters as well as to generate recommendations for process optimization in regional institutions. The aim of the study - to analyze the effectiveness of the regional program and develop proposals for the further development of rehabilitation services in the Astrakhan region. Materials and methods – the object of the study is the main indicator indicators of the regional program \"Optimal medical rehabilitation for health restoration (Astrakhan region)\" (hereinafter referred to as the Program) for the period from 2022-2030, namely, the share of cases of medical care for medical rehabilitation at the expense of CHI funds separately in inpatient and outpatient settings, as well as the share of medical rehabilitation departments, equipped with modern medical rehabilitation equipment, in comparison with the planned indicators for 2022. Results and discussion – the fulfillment of the plan task in the conditions of a round-the–clock hospital in 2022 in the subject amounted to 106.9%, in the conditions of a day hospital – 94.8% (on average 100.9%), which indicates that the first indicator indicator of the Program was exceeded. Planned indicators in outpatient settings were fulfilled in 2022 by only 58.4%. The reason for non-fulfillment of planned volumes was: 1) insufficient number of outpatient departments of medical rehabilitation; 2) the work of outpatient departments during an incomplete calendar year. In the subject in 2022, two departments of medical rehabilitation out of 5 planned until 2025 (i.e., in fact - 40%) are equipped with modern rehabilitation equipment, which indicates the implementation of the third indicator indicator of the Program. For objective assessment of the functional status of prostatic tissue, the «Kalium Parameter of Laboratory analysis, Evaluating in Ejaculate (KAPLEE)» was offered. KAPLEE is the concentration of K+ ions, measuring with the help of potentiometry in 42 ml of water solution, which contains 1 ml of ejaculate, 39 ml of distilled water and 2 ml of special buffer solution. The aim: to check the hypothesis that the deviation of KAPLEE from the normal range towards reduction or towards increase is the determinant of less efficacy of pumpkin seed oil (PSO) in patients with chronic prostatitis 3B (CP3B). Materials and methods: In trial 51 patients with the diagnosis of CP3B, who received PSO 5400 mg a day (4 capsules 3 times a day) for 30 days as monotherapy, took part. The study included 2 stages. During first stage each patient evaluated the efficacy of this pharmacotherapy in his case; it varied from 1 point (absolute non-efficacy) to 5 points (absolute efficacy). Then each patient ejaculated into container, and the concentration of KAPLEE was measured. Thus, the data of each patient were placed in two-axial system of coordinates: axis of abscisses was the concentration of KAPLEE, axis of ordinates was the mark of efficacy. Then the cluster analysis of these data was made. Results and conclusion. The extract preparation of shoots was obtained according to the well-known technological scheme: extraction of raw materials, purification of the extract, thickening of the extract. Ethyl alcohol of 70% concentration was chosen as the extractant, which is associated with the extraction of alcohol of this concentration of flavonoids - active biologically active substances of the raw material. The finished product was a thick liquid, greenish-brown in color with a specific odor. The yield of dry residue in the thick extract of shoots of Lespedeza bicolor was 22,2 ± 5,78%, the content of flavonoids, in terms of quercetin, was 1,98±0,027%. To study the local anti-inflammatory effect of the extract of the shoots of Lespedeza bicolor, studies were carried out to develop the composition and biopharmaceutical evaluation of a soft dosage form - an ointment with an extract of the shoots of Lespedeza bicolor. When biopharmaceutical evaluation of the release of the resulting ointment (gel) by diffusion through a semi-permeable membrane into an aqueous medium, it was determined that the maximum release of biologically active substances (flavonoids) from the gel is observed after 10 minutes. The introduction of an additional antimicrobial component, streptocide (sulfanilamide), into the composition of the developed gel with an extract of Lespedeza bicolor is substantiated. The study of the anti-inflammatory activity of gels with an extract of Lespedeza bicolor and gels of its combined form with streptocide was carried out on a model of acute exudative inflammation (formalin edema). We hypothesized that pumpkin seeds and PSO contain pharmacologically active substances that are able to influence on the rate of oxygen consumption (ROC) by the prostate tissue, which may be a determinant of their effectiveness in men with CP. The aim: to study the effect of eating pumpkin seeds, containing PSO, on the ROC of the prostate tissue in white outbred rats. Materials and methods. The control group consisted of 16 outbred male rats. The experimental group consisted of 6 white outbred male rats, which on the eve received, in addition to the standard diet, pumpkin seeds without restriction. Thus, 22 adult outbred male rats were used in the study. The ROC of the prostate tissue was measured by an optical sensor of dissolved oxygen with a DKTP-03 thermoelectric converter. A prostatectomy was performed under anesthesia. Immediately after that, prostate tissue weighing 0.5 g was placed in a beaker, filled with 15.0 ml of 0.9% sodium chloride solution at a temperature of 36.6 °C. An optical oxygen sensor DKTP-03 connected to an oxygen analyzer \"Expert-009\" was placed in a beaker and the tightness was created. The analyzer showed the concentration of dissolved molecular oxygen (CDO2) in a 0.9% solution of sodium chloride, expressed in mg / l. We recorded the CDO2 readings every minute from the beginning of the experiments (0 minutes) to their end (60 minutes).
