The hip abductors are known for stabilising the trunk and hip while walking, keeping the lower limbs in place, and transmitting stresses from the lower limbs to the pelvis. Methods: At Life Kare Hospital in Amritsar, India, an observer blinded randomised pilot trial was conducted. The experimental group received hip abductor strengthening coupled with conventional rehabilitation (n=10), while the control group received standard rehabilitation alone (n=10). Performance-based outcomes such as the timed up and go test, single leg stance test, six minute walk test, knee extensor strength, and hip abductor strength were used to assess physical function over the course of a year. Result: The study had eighteen participants, with an average age of 62.3 ± 5.7 years (10 males and 10 females). When compared to normal rehabilitation alone, improvement in hip abduction strength, single leg stand test was superior in the hip abductor strengthening group at 3 months and 1 year. Conclusion: In both the single leg stance test and the six minute walk test, hip abductor strengthening demonstrated superior results. Following complete knee replacement, hip abductor strengthening exercises have the potential to improve physical function.","PeriodicalId":151163,"journal":{"name":"National Journal of Clinical Orthopaedics","volume":"54 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"117295868","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Total No of cases: 50 Patients with intertrochanteric fracture femur satisfying the inclusion criteria, who required surgical intervention, were worked up clinically and radiologically. All patients selected for the study were examined according to protocol, associated injuries, if any, were noted and investigations carried out in order to evaluate fitness for anesthesia. Result: Age distribution pattern of the patients. Average age -79.5 years, youngest patient-70 years and oldest-90 years. Increase in average Harris Hip Score at 1 month, after 6 months and after 1 year follow up. Conclusion: Primary hemiarthroplasty provides a stable, pain-free, and mobile joint with acceptable complication rate as seen in our study. The meticulous reconstruction of the posteromedial calcar area with bone graft taken from neck of femur is crucial for stable implantation of the prosthesis. Early mobilization, less hospital stay and excellent stability offered by hemiarthroplasty makes it a promising method to deal with the challenging problem of comminuted intertrochanteric fractures in the elderly population.","PeriodicalId":151163,"journal":{"name":"National Journal of Clinical Orthopaedics","volume":"38 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122839510","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Intertrochanteric fractures form around half of the total hip fractures in the elderly and pose a number of management dilemmas depending on the fracture configuration and status of the bones. There are numerous implants available for the internal fixation of these fractures ranging from dynamic hip screw which can be combined with trochanteric stabilisation plate; locking plates; intramedullary implants such as proximal femoral nail (PFN), trochanteric femoral nail (TFN) and Gamma nail. Materials and Methods: The fractures were classified according to BOYD and GRIFFIN classification system. 40 patients of intertrochanteric fractures fulfilling the inclusion and exclusion criteria, were managed surgically using Proximal Femoral Nail. The results were analyzed according to age, type of fracture, operative details and functional outcome using KYLE’s criteria. Results: Totally 40 patients with intertrochanteric fractures were operated with PFN and were included in the study. The mean age of the patients was around 68 years with minimum age being 52 years and the maximum being 88 years. The mean duration of operation was recorded to be 54.59 min. Complications in this study were seen in 3 patients (8%). Complication was surgical site infection 2 patients (5%), greater trochanteric fracture 1 patient (2.5%). Conclusion: The proximal femoral nail acts as a buttress to prevent medialisation of the shaft & provides more effective load transfer. It has a chance of failure in the comminuted and severely osteoporotic fractures. It is superior implant for both stable & unstable fractures. It also has advantages of having decreased blood loss, decreased operating time, decreased complication rates.","PeriodicalId":151163,"journal":{"name":"National Journal of Clinical Orthopaedics","volume":"97 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124670377","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The Latarjet’s procedure that involves transfer of the coracoid to tuck the glenoid bone defect is a popular surgical procedure and this study was undertaken to evaluate the efficacy of Latarjet’s procedure in the management of recurrent shoulder dislocations after a failed index surgical procedure other than Latarjet’s procedure. Materials and Methods: A total of 5 patients were included in the study (3 male; 2 female). All the patients were operated using the Latarjet’s procedure and a follow up was done at regular intervals. The evaluation at follow up included serial radiographs along with outcome assessment parameters such as subjective shoulder score, Constant-Murley score and VAS score. Results: There were no cases of redislocations or subluxations at the end of follow up. No revision surgery was needed. The mean shoulder value improved from 61% to 83% postoperatively at 1 year follow up (( p < 0.001)) and the relative Constant -Murley score improved from 75% preoperatively to 85% postoperatively at the end of 1 year of follow up(>0.05). It is defined as pain that persists longer than 12 weeks and is often attributed to degenerative or traumatic conditions of the spine. Fibrositis, inflammatory spondyloarthropathy and metabolic bone conditions are also cited as causes. Although acute low back pain has a favorable prognosis, the effect of chronic low back pain and its related disability on society is tremendous. Unlike acute low back pain, chronic low back pain serves no biologic purpose. However, it is a disorder that evolves in a complex milieu influenced by endogenous and exogenous factors, and it alters the individual‘s productivity to an extent beyond what the initiating pathologic dysfunction would have. The purpose of this study is to determine the relative efficacy of core stabilization exercises compared to general low back exercises in reducing chronic low back pain. core stabilization exercise Study method: This a short term study, where thirty chronic low back pain patients with pre-defined selection criteria were selected and assigned into equal experimental groups as group A (core stabilization exercises) and group B (general back exercise). The pre and post test outcomes of pain intensity are marked on the VAS. Then statistical comparison is drawn between the experimental groups mean values by using unpaired‗t‘ test. Results: The study results show that the pain intensity mean values have decreased to significant extent after the treatment period. The mean pretest values of both core stabilization exercises and general back exercise groups before the treatment are 5.5 and 5.2 respectively which have been decreased to 1 and 1.7 after the treatment. Conclusion: Both the treatment procedures have shown an effective outcome, but core stabilization exercises have proven to be more effective than the general back exercise statistically.","PeriodicalId":151163,"journal":{"name":"National Journal of Clinical Orthopaedics","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116309389","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and Methods: The proposed study is a prospective non randomized control study on the management and treatment of clavicle fractures centered in K.R Hospital attached to the Government Medical College and research institute, Mysore during the term between January 2019 to December 2019.A Total of 31 Cases of Patients underwent internal fixation by MIPO using Pre-contoured locking compression plate. Patients were functionally assessed at the end of follow up period as per the CONSTANT MURLEY Shoulder outcome scoring system. Results: Out of 31 patients, 87.1% of patients were below the age of 40years in our study with Males (87.1%) and Females (12.9%). Most of the patients were operated within 3 days and Right sided fractures were more common than left sided (54.8% vs 45.2%).Average time of union was 13 weeks. Most of the cases did not have any complications except for hypertrophic scar in 2 patients, implant prominence in 3 patients, superficial infection in 1 patient. Conclusion: Closed biological fixation using MIPO technique with locking Pre-contoured plate can preserve the blood supply of the fracture site and minimize soft-tissue related complications. Although it needs surgical expertise and proper technique for better results. MIPO technique with locking reconstruction plate is a feasible and worthwhile alternative for DMCFs.","PeriodicalId":151163,"journal":{"name":"National Journal of Clinical Orthopaedics","volume":"6 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123829385","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
