Thavamani","doi":"10.26611/101512217","DOIUrl":"https://doi.org/10.26611/101512217","url":null,"abstract":"BACKGROUND: \u0000The hemodynamic response associated with laryngoscopy and tracheal intubation may be harmful to certain patients .The laryngeal mask airway avoids the need for laryngoscopy and provides positive pressure ventilation of the lungs in appropriate patients. \u0000AIMS AND OBJECTIVES: \u0000This study compares the hemodynamic response of laryngoscopy and tracheal intubation to that of laryngeal mask airway insertion in hypertensive patients. \u0000METHODS: \u000060 Hypertensive patients between 40 to 60 years of either gender of ASA Grade II were randomly allotted to one of the groups of 30 each, group ET and group LMA .LMA insertion or tracheal intubation was performed after induction of anesthesia with Thiopentone and muscle relaxation with succinyl choline. Anesthesia was maintained with sevoflurane and nitrous oxide, oxygen the heart rate, systolic BP, diastolic BP, mean arterial pressure and rate pressure product were measured after induction and immediately after insertion / intubation and then after 1, 3, 5 minutes. \u0000RESULTS: \u0000There was very highly significant difference (p < 0.000) in mean increase in heart rate (38.23% in group ET versus 28.26% in group LMA). The increase in arterial pressure were also significant. The systolic BP increased 40.16% in group ET compared with 37.60% in group LMA (p < 0.000). The diastolic BP was also seen increasing by 22.73% and 14.23% in group ET and group LMA respectively. \u0000The MAP and RPP values were maximum after airway instrumentation however we found that values after LMA insertion were significantly lower when compared to tracheal intubation for the first 3 minutes. \u0000INTERPRETATION AND CONCLUSION: \u0000The data suggest a similar, but attenuated pattern of response associated with mask insertion in comparison to laryngoscopy and intubation .Hence use of LMA may therefore offer some advantages over tracheal intubation in anesthetic management of patients where avoidance of pressure response is of particular concern.","PeriodicalId":118187,"journal":{"name":"MedPulse International Journal of Anesthesology","volume":"31 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122102328","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Numerous strategies are currently invoked to manage the requirements of cooling, temperature maintenance, and rewarming patients during cardiac surgical procedures.Aim: To study the rate of cooling and warming at different sites of the body following extracorporeal cooling and rewarming in patients during open heart surgery.Materials and Method: The present study was conducted on fifty patients undergoing elective open heart surgery at Tertiary Care Hospital in Jaipur city. Fifty patients belonging to both sex and different age groups were studied. The patients were divided into two groups. Group I consisting of patients in which Nitroglycerin was not used as vasodilator during rewarming on Cardiopulmonary bypass and Group II where Nitroglycerin was used.Results: During bypass stage, the Skin, Rectal and Nasopharyngeal temperatures fell because of hypothermia. In both groups the rectal temperature fell slowly. The fall in skin temperature was maximum whereas nasopharyngeal temperature showed an intermediate fall.Conclusion: The temperature of heart determines the degree of protection afforded to it during coronary occlusion by aortic cross clamping. Difficult tracheal intubation accounts for 17% of the respiratory related injuries and results in significant morbidity and mortality. There are numerous literature on comparison between these two supraglottic airway devices with contradictory results. Objective: To compare LMA Proseal and I-gel for ease of insertion based on Modified Scheme of Lund and Stovener, jaw relaxation based on Young’ criteria, number of attempts for insertion in adult patients undergoing elective surgeries. Methodology: It is a RCT involving Sixty patients, scheduled for various elective surgical procedures undergoing general anaesthesia belonging to ASA class I and II were included in the study. The patients were divided in two groups Group P-proseal, Group I-I gel 30 each. Results: The mean age in group LMA-P and group I-gel was 30.9 and 31.23 years respectively. Majority of cases were from Grade I in both P (66.7%) and I (83.3%) groups. The attempt of insertion was statistically significant between the two groups (p<0.05). Time required in Group I is less compared to group P in our study.
