Herein, the authors report a middle-aged patient with intracranial subarachnoid hemorrhage and intracerebral hematoma after blunt cephalic trauma. The trauma led to PSA in the A2 segment of the left anterior cerebral artery (ACA). The patient was treated with simple coil embolization to prevent rebleeding in the acute phase. After 20 days, angiography revealed recurrence of the aneurysm. Thus, PED Flex was implanted in the second phase. A 6-month postoperative follow-up revealed the complete disappearance of posttraumatic PSA, and the ACA remained unobstructed. Abstract

Traumatic intracranial pseudoaneurysm (PSA) is a rare yet dangerous subtype of cerebral aneurysm. The pipeline flex embolization device (PED Flex) has recently been applied to treat intracranial PSA.
