Diagnosis is based on a good understanding of the mechanism of trauma, a focused clinical examination, and judicious use of imaging, particularly MRI. Treatment may be conservative or surgical, depending on the degree of instability. Abstract

Injuries of the distal tibiofibular syndesmosis are a common cause of ankle pain in athletes. Although less frequent than lateral sprains, they are associated with greater functional impact, prolonged recovery time, and increased risk of complications if not properly diagnosed. Diagnosis is based on a good understanding of the mechanism of trauma, a focused clinical examination, and judicious use of imaging, particularly MRI.
