Extraocular manifestations, particularly on the nose, are exceptionally uncommon. A 91-year-old female patient presented to the plastic surgery clinic with a painless nodular mass on the nose. A punch biopsy confirmed sebaceous carcinoma, prompting a full-thickness lateral alar resection with immediate reconstruction using a superiorly based folded nasolabial flap. The surgical wound healed without complications, achieving favorable functional and aesthetic outcomes.
