Abstract

Frailty is characterized by decreased physical function and physiological reserve, which impact quality of life and predispose individuals to risk for or difficulty recovering from illness and injury. Although older adults are at risk for greater levels of frailty, it is not strictly age-dependent. Frailty assessment tools commonly assess the presence and severity of frailty. NICHE organizations are integrating frailty assessment to identify patients with the greatest need for prevention and management of geriatric syndromes and the application of care processes and resources, such as care by geriatric resource nurses.
