Abstract

Percutaneous coronary intervention (PCI) for chronic total occlusion (CTO) lesions with ambiguous proximal cap remains challenging. We present a successful CTO-PCI case involving proximal cap ambiguity, treated by combining balloon-assisted subintimal entry (BASE) and tip-detection antegrade dissection and re-entry (TD-ADR) techniques. The BASE-facilitated TD-ADR technique is a novel antegrade CTO-PCI approach and has the potential to enhance the safety and efficacy of antegrade CTO-PCI procedures.
