Abstract
A 4-year-old girl presented for a dental appointment at the Special Care Dentistry Center, brought by her paternal grandmother complaining of dental hypersensitivity. The patient was born prematurely, with low birth weight, and congenital syphilis. She presents with deafness, prominent frontal bossing, and so far, has shown normal neuro-motor development. She has primary dentition with generalized enamel structure alterations, characterized by yellowish teeth, hypoplastic teeth, enamel prominences, and anterior open bite. Radiographic images and dental history revealed the congenital absence of mandibular second premolars and notched incisors. Interstitial keratitis, which completes the Hutchinson's triad, has not been investigated yet. The treatment plan includes preventive care, photobiomodulation protocol for dentin hypersensitivity, molar sealings, direct resin restorations for aesthetic purposes, and occlusal follow-up of permanent dentition. Congenital syphilis is a preventable disease, and its presence reflects a failure of the prenatal care delivery system, as well as syphilis control programs.
