La méthode d'Ilizarov pour l'allongement et la correction du cubitus dans la main bote radiale

Abstract

La technique révolutionnaire de traitement des fractures et de leurs complications par la méthode d'Ilizarov trouve également son application dans le traitement des malformations congénitales. La compactotomie et la distraction des segments osseux de 1 mm par jour, pratiquées selon la méthode d'Ilizarov, provoquent également un allongement progressif induit des parties molles contiguës à l'os. Le patient que nous avons opéré était un enfant de 12 ans présentant une main bote radiale bilatérale associée à une cardiopathie congénitale et des malformations génito-urinaires. Il avait subi deux ans auparavant (1982) une intervention de centralisation du poignet droit sur le cubitus, avec intervention plastique cutanée et aponévrotomie du versant radial de l'avant-bras, et il présentait une incurvation résiduelle du cubitus de 40° environ. Par la méthode d'Ilizarov, nous avons obtenu la correction complète de l'axe et un allongement du cubitus, des nerfs, des artères, des veines et des muscles de 7,5 cm par rapport au cubitus opposé. En 1985, on a pratiqué une intervention de pollicisation de l'index de la main droite. Actuellement, la déformation anatomique et esthétique du membre supérieur droit est très atténuée, la main atteint l'épaule homolatérale, l'avantbras est proportionné au bras et à la main.

In this way it is possible to obtain progressive lengthening of a limb or bone fragment. Bone stumps are fixed with crossed Kirschner wires, and such as we can obtaine at the same time a lengthening of nerves, vessels, tendons, muscles ; and that, in one of his patients, Ilizarov succeded in reaching an elongation until 25 cm ! Our patient is a 12 years old boy, affected by bilateral radial club hand, congenital heart and kidney diseases. After both wrist centralization requiring soft tissues release, we established that the right forearm showed persistant shortening and radial bowing of the ulna. So we used the Ilizarov's method, mending both deformities and reaching an elongation of the ulna of 7,5 cm. A remarkable both functional an aesthetic improvement of the forearm has been also obtain without any loss of function of soft tissues, such as throphic troubles, functional loss of vessels, nerves, tendons, muscles and skin.
