This case underscores the importance of considering acute MI in any patient with chest pain, regardless of age. Cardiac imaging is essential for confirming and guiding the diagnosis. We present a case of a very young adult with acute myocardial infarction with nonobstructive coronary artery (MINOCA). Furthermore, after investigation, we identified 7 genetic mutations predisposing the patient to coronary artery thrombosis. MI has a complex etiology involving a combination of genetic factors and internal and external environmental risk factors. Abstract
