More recently, dual-energy CT has enabled the functional evaluation of the myocardium and coronary arteries. Herein, we present the case of a 39-year-old man with chest pain who was referred for cardiac CT to investigate ischemic heart disease. CT revealed significant stenosis of the left coronary artery and severe stenosis of the left circumflex branch. Dual-energy imaging, myocardial perfusion assessment, and delayed contrast assessment further revealed indications for revascularization of the left anterior descending artery. This experience shows that dual-energy CT allows detailed myocardial perfusion and viability assessments, aiding lesion identification and guiding revascularization procedures.
