Despite therapy, the patient's symptoms worsened, and his general condition started worsening with, fever, and leukocytosis, requiring hospitalization and IV therapy due to secondary meningitis following postoperative endophthalmitis. Abstract

A 61-year-old man exhibited hyperacute endophthalmitis due to Proteus mirabilis after a pars plana vitrectomy. During the index examination (24 hours after surgery), he showed visual acuity (VA) of hand motion, biomicroscopy revealed the presence of an edematous cornea, Tyndall++++ and fibrin membrane, with vitritis and impossibility of visualizing retinal details. Treatment with intravitreal injections was administered.
