This study investigates the relationship between ULR and clinical outcomes in patients with cerebral venous sinus thrombosis (CVST).</p><p><strong>Patients and methods: </strong>A total of 636 patients with CVST were included in the study and randomly divided into a training set (n = 445) and a testing set (n = 119) in a ratio of 7:3.Logistic regression analysis was employed to analyze the risk factors for poor outcomes. The nomogram was established using the training dataset, and its predictive performance was assessed with the testing dataset.</p><p><strong>Results: </strong>ULR accurately predicted poor outcomes in CVST and was linked to a higher likelihood of poor outcomes (OR=1.015, 95% CI: 1.003-1.026, <i>P</i> = 0.013). Age, infection, intracranial hypertension, coma, and intracerebral hemorrhage were independent predictors of poor outcomes in CVST. Additionally, a new nomogram incorporating ULR was constructed to predict the risk of poor outcomes in CVST patients. The nomogram demonstrated good accuracy and reliability, as shown by the receiver operating characteristic curve, calibration curve, and decision curve analysis.</p><p><strong>Conclusion: </strong>ULR independently forecasted poor outcomes in patients with CVST. Abstract
