It occurs in women of reproductive age, commonly with a history of breastfeeding. It is manifested by the formation of abscesses or tumors. For the diagnosis, a biopsy is required and to rule out, in addition to infectious processes, autoimmune diseases. Treatment is controversial, antimicrobials, steroids, immunosuppressants, biological therapy, and surgery have been used. We present the case of a 40-year-old woman with a typical picture of granulomatous mastitis, with failure to the usual therapeutic options, was managed with baricitinib, observing an excellent therapeutic response.
