The design of the electronic assessment form for child speech therapy and child occupational therapy is needed to support the implementation of electronic medical records in the clinic. This study aims to identify the data requirements needed to design, design the user interface of the electronic assessment form for child speech therapy and child occupational therapy and test the design. Methods: Descriptive research with a qualitative approach. The object is the conventional speech therapy and occupational therapy assessment form, the subjects are medical recorders, child speech therapists and occupational therapists and pediatricians. The design method uses User Centered Design (UCD). Results: The design consists of data requirements, use case diagrams and user interface designs, namely menus/dashboards, services, and assessment forms for child speech therapy and child occupational therapy.
