Abstract
This paper presents a mobile solution for the management of chronic diseases, i.e. hypertension and diabetes. This solution facilitates the exchange of health information between patients and pharmacists, supports wellness decision making, and assists behavioral interventions for enhanced self-management and healthcare delivery.
